Chapter 1: Administration Block
At the start of the game, Miles Upshur, an investigative reporter, is seen driving to Mount Massive Asylum for the Criminally Insane to discover and expose its secrets after receiving an anonymous tip that potentially criminal acts have been occuring in the asylum.
Chapter 2: Prison Block
After being sedated and dragged by Father Martin, Miles wakes up in a locked padded cell, with the phrase “Rest in Peace” written in blood on the wall. When he approaches the door, one of the patients unlocks the door for him and quickly leaves. Miles leaves the cell and encounters The Twins, mysterious Variants, standing on the other side of the prison balcony behind iron bars.
Chapter 3: Sewer
The chapter starts as Miles goes underground through a hole at the Prison Block. He finds a broken wall and uses it to enter the sewers; immediately afterward, he goes through several chambers, enters a pipe, and spots The Walrider as it passes through a tunnel.
Chapter 4: Male Ward
Miles enters the Male Ward through a staircase from the Sewer. Once he enters, he finds a metal storage unit, containing propane tanks, and pushes it out of the way, revealing a vent. He travels through it and finds a Variant in a straitjacket.
Chapter 5: Courtyard
Miles escapes from the Male Ward, and exits through the same exit that Trager had jokingly showed him before taking him to the Male Ward.
Chapter 6: Female Ward
After entering through a window from the Courtyard, Miles sees Father Martin on a ledge, where the floor has fallen through, like many areas in the Female Ward. As he travels into the ward, he finds a staircase with arrows drawn in blood indicating to travel up.
Chapter 7: Return to the Administration Block
After leaving the Female Ward, Miles will find a Variant telling him, “Only one way out, only one way,” indicating for him to keep moving. After pushing through two areas of narrow passage, Chris Walker will appear at the end of the hall Miles is moving down.
Chapter 8: Underground Lab
After Father Martin commits self-immolation in the chapel of the Administration Block, Miles enters the elevator, rigged by Martin to go to the Underground Laboratory. It passes the Administration Block, then, continues to the lab. Upon entering the lab, Miles discovers dead guards at the front desk, where Martin had shown him the security footage of The Walrider.
