Swimmer
I guess it is intended that you go into the big lake in the south-west to get it, however I got the achievement by simply walking around the map.
Fire
Walk in the camp fire in the little village in the north (there is also a note on the table).
Abandonend house
(Credit for this goes to Charmed76 who helped me getting this achievement.)
Go to the house at the bottom right of the map.
End Game
Collect all 7 notes, then the screen will fade into black and you’ll get it.

(It is possible that you can only pick it up if you have all other notes collected. Also this note is a bit glitchy, I needed some seconds so I could pick it up.)
