The following window will list your government type unter the name “constitutional form”.
If you want to change it, click on “more”.
Here you can see all the available government types (except for anarchy, it will be explained later).

Anarchy isnt a government form you can simply choose. Your country has to be very destabilized (stability rating under 10%) and it will automatically turn into anarchy.
