Also known as ‘the half that has players and items but no more bosses’This is basically just the 2nd half to the boss page, containing all the loose ends and non-boss-related ideasAs usual, the disclaimers are that A. This is not a guide and is just a place to put ideas and B. Items aren’t sprited, even though some characters do have makeshift sprites to give a better idea. The items likely won’t have sprites for a while just due to how many there are, as well as my lack of familiarity with spriting TBoI stuffAnyways, on with the page 🙂

This page is basically just containing the two major factors absent from the first half (the bosses); those being characters and items. The characters will come first, with a basic list of unlocks beneath them, and the items will come after, including the more in-depth descriptions for the items listed alongside characters.

Mulligan
Appearance: A fly-infested Isaac, similar to the enemies of the same name.

-The Player’s tears are replaced with special Blue Flies. The player can, however, only spawn 6 of their own blue flies before being unable to spawn more. Other fly-spawning items, like Guppy’s Head, are unaffected by and do not affect the limit.
–The Player’s special flies will take on certain tear properties, but do not work with others. This effect does not transfer over to other fly-spawning items.
–Multi-shot items, like 20/20, Inner Eye, and Mutant Spider will increase the amount of flies spawned, and will increase the soft cap for player-spawned flies by 2, 4, and 6 respectively

-Due to the special flies only picking up some tear effects, certain items such as Brimstone and Mom’s Knife do not work with this character
–Some other items have partially-active effects, such as Ipecac/Dr. Fetus making flies explode upon damaging enemies, and Parasite/Cricket’s Body/Haemolacria causing flies to spawn tears upon damaging enemies
-Shot Speed and Range do not affect this character very much as they lack actual projectiles.
–However, combined with items that give unique tear properties like splitting tears, the spawned tears can be affected
-Originally, this character was unlocked by getting the Lord of the Flies achievement, though was changed due to Guppy being the more fly-focused character and a transformation that was more sought after
-Hive Mind is, as expected, amazing for this character as it essentially doubles their damage at all times

Siren
Appearance: An Isaac-sized version of one of the Sirens seen during the first phase of the Kraken fight
[Unlock: Use the Chaos Card on the Kraken during phase 2. After going into the back room with the boss item and the door to Depths II, exit the room back into the boss arena. One of the Sirens will have returned to the rock pedestals. Walk up to them and Siren is unlocked]

Berserker
Appearance: A buff Isaac with lumpy eyebrows and a large ingrown tooth. The design has some similarities to the Spun transformation

The rock wave will stop upon hitting a rock or a gap, but will also break the rock or fill a gap so the player cannot be softlocked. This attack cannot open room doors or open bedrooms, however.
–Any active items encountered in a run, when picked up, will be used alongside the rock wave attack. The player can switch between an active item and the rock wave attack with control

–This means that certain items, like Brimstone, are actually a hindrance to the player as all it does is decrease your tear rate for no benefit

Guardian
Appearance: A blindfolded, winged Isaac. The blindfold is similar to Lilith’s and the challenge blindfold; covering the player’s eyes so they cannot shoot tears

The Dreamer
Appearance: An Isaac with colours based on his photo-negative colours. His eyes are a pale white and seem sad, and his mouth seems to be absent
[Unlock: Defeat the Sandman as any character and go through his portal. Dreamer will be unlocked alongside whichever character-specific item is also unlocked upon the Dreamer’s defeat, but [i]only[/i] if the player takes the portal and doesn’t go to the Void.]

-The Dreamer’s colour palette for the eyes, skin and tears are actually directly taken from a photo-negative Isaac, with the shades inverted so the colours match the regular shading style of the game
-The warped face of the Dreamer looks almost like the Bony enemies and the Forgotten, suggesting the Dreamer’s face appears almost skeletal
-Unlocking the Dreamer requires the player go go through the Sandman’s portal and NOT go to the Void. If the player goes to the Void instead, they will need to re-fight the Sandman again. This is because the Dreamer is linked to the player viewing the ending, not the boss itself.

Tofu Boy
Appearance: A reference to Tofu Boy from Super Meat Boy, complete with the sunken eye sockets and pale skin.
[Unlock: It’s Complicated]
The player needs to complete a few things prior to attempting to unlock Tofu Boy:
-Complete a Meat Boy or Bandage Girl
-Defeat Mom’s Heart in Hard Mode as every character
-Unlock the ??? and Void stages
Once all of these tasks are completed, they must play a run and complete the following criteria in a single run:
-The player must play on Hard mode
-The player must find an I AM ERROR room on any floor before Depths II
–While in this room, the player must bomb the shopkeeper, and then wait 5 minutes for a chime to play, and then take the trap door
-Complete the game up to Mom and defeat Mom. Regardless of the time, the Boss Rush is unlocked.
–Complete the Boss Rush. Being teleported out of the Boss Rush will void the attempt immediately
-Complete the game up to Mom’s Heart/It Lives!. Regardless of the time, the door to ??? will appear
–Go to ??? and defeat its boss, and then take the Void portal
-Go through the Void and defeat its boss. A choir will play, square blobs of white ‘goo’ fall from the ceiling in random places and, after the ending plays, the player will have unlocked Tofu Boy (And subsequently be called out for unlocking a joke character)

Effect: Grants an aura around the player. This aura has a chance to charm enemies that get close.

Effect: Tears up substantially, but reduced range and damage and tears are slightly inaccurate. Shots are bubble-like in appearance and decelerate after travelling for a little over a second.

Name: Rainmaker
Tooltip: ‘Rain down tears’
Type: Passive Item
Effect: Attacks will now summon a cross marker on the ground that can be positioned. After roughly a second the marker disappears and several tears will rain down from the sky above where the marker was. Replaces normal tears.

Type: Passive Item
Effect: Sometimes replaces Isaac’s tears with larger ‘absorbing’ tears. These tears move slower and are significantly larger, but have a guaranteed Lost Contact-like effect where they can block hostile projectiles.

Effect: May sometimes turn the player’s projectiles into a triple shot of musical notes instead. Effect has a 2 second cooldown, regardless of chance.

Effect: Replaces tears with musical notes. Unlike other note-like projectiles, these ones deal damage rapidly and can hit the same enemy several times, but cannot charm enemies. Tears and damage down.

Effect: Taking damage or eating a pill will cause the player to spew out a cluster of projectiles, similar to Monstro’s Lung, that also leave lingering creep. All tears also have a small chance to inflict poison.

Tooltip: ‘Numb feeling’
Type: Passive
Effect: Whenever damaged by an enemy, the player will release a series of short-range needles, and additionally has a chance to not take damage. Player also deals contact damage.

Effect: Tears down, but projectiles have piercing and split into 4 projectiles after piercing an enemy.

Effect: Will grant Spun! immediately. Increases 3 stats randomly, and decreases one stat more drastically, though the decreased stat cannot be the same as one of the buffed stats.

Effect: Grants similar buffs to the Strength card that last until the player leaves the room.

Type: Passive
Effect: Mild buffs to damage and range. Attacks have a chance to be golden, which gives enemies a golden aura. Enemies with golden auras can be instantly killed by Midas Touch (aside from bosses who just take major damage) and, upon death, explode into 1-3 coins.

Name: Junkie
Tooltip: ‘Needle Shot’
Type: Passive
Effect: Tears up. Has a small chance to launch a needle instead of a tear that deals 1.5x tear damage and causes afflicted enemies hit by the needle to take 1.2x damage for a short time. Once applied to an enemy the chance for needles to be able to re-apply the damage vulnerability again is halved each time it is successful.

Name: Hallucinogens
Tooltip: ‘Are they really there?’
Type: Passive
Effect: When damaged, there is a chance to instantly kill any regular enemies in the room, calculated individually per enemy. Also activates with the Lost’s Holy Mantle. Insta-killed enemies also have a chance to drop soul hearts.

Effect: Tears down significantly. Player launches shots in clusters every shot, similar to Monstro’s Lung, that leave creep where they land.

Effect: Significantly buffs all of the player’s stats for the current room. However, upon clearing the room, the player will die after 3 seconds.

Effect: Mass room damage, dealing enough damage to instantly kill most regular enemies. Can also instantly kill the Devil and Isaac, but doing so will also destroy this item in return.

Name: Ascension
Tooltip: ‘Stats up and flight… for a price’
Type: Passive
Effect: Grants flight and increases speed, shot speed, tears and damage. Grants 3 soul hearts and replaces all future Deals with Angel Rooms. However, all of the player’s red and black hearts are turned into soul hearts, and all future health-ups are also turned into soul hearts, similar to ???
Unlock: Defeat Isaac as the Guardian
Name: Exodus
Tooltip: ‘Wash them away’

Charge: 5 Rooms
Effect: Opens all exits to the current room, deals minor damage to all enemies in the room, and leaves damaging blue creep over the whole room.

Name: Faded Picture
Tooltip: ‘Does… something?’

Charge: Single-Use
Effect: Pseudo-resets the current floor. The reset version is entirely monochrome, as are the enemies. Enemies are generally stronger, pickups are more common, items are re-rolled, and Deal/Angel Rooms are guaranteed after defeating the boss.

Effect: Allows the player to jump. Upon landing, the player creates 2 rings of rock waves around themselves that deal double player damage.

Name: The Tunneler
Tooltip: ‘The ground is moving’
Type: Active, Familiar
Charge: 3 Rooms
Effect: On use a familiar is spawned, called the Tunneler, which burrows through the ground after enemies. It produces small rock waves while moving that deal minor damage, and the Tunneler’s own contact damage is based on the player’s tear damage.

Effect: Spawns a familiar based on the Fallen boss. Charges at enemies before releasing a + of brimstone lasers.

Effect: Summons doors on all the walls where doors could appear, or uses existing doors. Golden brimstone lasers will be fired from all the doors for a few seconds, and then the summoned doors will all disappear again.

Effect: Does a random active item effect every time it is used. Double-charge items will cause it to fulfil 2 random effects.

Effect: While close to enemies, a hidden meter fills up. Once full, shadowy tendrils will lash out at nearby enemies for a few seconds, dealing massive damage.

Name: Tunneler II
Tooltip: ‘Become the Tunneler’
Type: Passive
Effect: Replaces the player’s face with a large mouth. Damage up and tear size up. If the player takes damage they instinctively burrow into the ground for a short time. After a second the player pops back up, creating a ring of rock waves that push enemies away and deal additional damage.

Name: Mask of the Dreamer
Tooltip: ‘The world is an illusion’
Type: Passive
Effect: Grants 3 soul hearts, grants a chance to ignore damage from attacks, shot speed and range up, but turns all of the player’s red hearts into soul hearts and removes all red heart containers, but doesn’t stop the player gaining more red hearts in the future.

Name: Brittle Key
Tooltip: ‘Brittle, but works’
Type: Trinket
Effect: Acts as a key. Every use halves the chance it’ll work next use, and if it fails to work the trinket will simply break. Used before any keys the player has, and cannot be given to key beggars.
Unlock: Beat the Boss Rush as Tofu Boy
Name: Brittle Heart
Tooltip: ‘Damage vulnerability, but more rewards’
Type: Trinket
Effect: Player takes a full heart of damage from all attacks, even when using damage resistance such as the Wafer. Drops from clearing rooms are more common, and also adds a chance for double drops.
Unlock: Defeat the Devil as Tofu Boy
Name: Brittle Soul
Tooltip: ‘Stronger mind, brittle body’
Type: Trinket
Effect: If the player is below 2 hearts any damage will kill them instantly, and if above 2 hearts damage will reduce them to only 1 heart and automatically make them drop this trinket. However, while equipped, the player gets drastic buffs to move speed, damage, tears, and shot speed.

Name: Dry Fly
Tooltip: ‘It’s stuck’
Type: Passive, Familiar
Effect: A fly stuck in a greyish block slowly orbits the player, blocking shots. After 25 projectiles hit it, it will permanently break out of its casing and orbit the player farther out than before, and significantly faster also. It also gains the ability to deal contact damage.

Effect: Places down a greyish mound of tofu. The tofu block can absorb up to 20 projectiles before breaking, or a single bomb, and acts like a physical object to hinder player and enemy movement. Cannot drop items like poop can when destroyed, however.

Effect: Certain rocks will be replaced with golden rocks. Golden rocks have small pieces of gold in them and, when destroyed, drop a small cluster of coins.

Name: Clump of Tofu

Effect: A familiar that builds up slowly the more Clumps of Tofu that are picked up. Stage 1 is an orbital that does little contact damage, Stage 2 has a face and lobs slow-moving projectiles, Stage 3 chases enemies and deal contact damage, but will run out of breath sometimes. Stage 4 deals massive damage, but turns back into Stage 3 after hitting an enemy once, recharging every room.

Name: Bloody Bow
Tooltip: ‘Eye for an eye’
Type: Passive
Effect: Grants 2 black hearts. Every time the player loses a full heart mass room damage is dealt based on the heart lost. 10 damage is taken for red and black hearts, and 20 for all other heart types. Black heart damage stacks with the 40 already dealt, and gold hearts are ignored.

Name: Zombie Eye
Tooltip: ‘Smells and looks awful’
Type: Passive, Familiar
Effect: Spawns a Zombie Eye familiar that bounces around the room, dealing contact damage. The familiar also draws most fly-like enemies towards it.

Type: Passive, Familiar
Effect: Closely orbits the player, blocking shots and possibly dealing contact damage. After being hit by 3 hostile projectiles it will stop orbiting the player and blocking projectiles and begin chasing enemies instead, dealing contact damage. Resets every room.

Name: Make-Up Palette
Tooltip: ‘Oh so pretty’
Type: Passive
Effect: Tears down. If no enemies in the room are currently charmed the player has a high chance to launch a charm tear. If an enemy is charmed, the player loses the ability to shoots charm tears but will receive small buffs to damage, shot speed, tears and range until the charmed effect wears off or the charmed enemy dies.

Name: Prison Shackle

Effect: Spawns a Frowning Gaper-like familiar that is chained at the centre of the room. It rapidly damages enemies that get close to it, and charges at enemies that get within its range.

Effect: Gives the player a random pill, card, trinket and treasure room item (excluding active items), as well as a weaker D8 effect. Every floor re-applies the D8 effect, re-rolls the treasure room item given to the player, and if the player has a trinket or card/pill it is randomised also. The special treasure room item does not remove items from the item pool and can re-apply items already picked up, and the randomised trinket can re-roll the Tick.

Effect: Once used in a room, if the player were to die in that room, they will instead be revived outside of the room with the same health they had when the item was used.

Effect: Defeating bosses or mini bosses will cause them to additionally drop a dime or a nickel, respectively.

Use: Infinite
Effect: Can activate certain on-damage effects on use without necessarily hurting the player, including invulnerability frames, on the first use in a room. There is a 10% chance to deal half a heart damage to the player for the first use. Every subsequent use adds another 40% chance to take damage, only resetting after clearing a room with enemies.

Name: Zombie Parasite

Effect: The player’s health is capped at 6 hearts maximum (3 for the Forgotten), and a speed down. However, the player will receive damage resistance akin to the Wafer, grants a soul heart, increases the player’s damage, shot speed and tears, and finally increases the chance for hearts to drop.

Name: Spectral Bird

Effect: Summons a bird familiar that attacks enemies. Every time it kills an enemy, it spawns another bird, capping out at 8 birds. Resets every room.

Effect: Summons a knife in front of the player that, when firing tears, will be launched forward and keeps travelling in a straight line until it hits a wall. After hitting the wall it will be lodged into it for a short time before returning to the player. The knife deals damage rapidly, both when in front of the player and when being thrown.

Name: Self-Defence

Charge: 6 Uses
Effect: Shoots a fast bullet that instantly kills regular enemies and deals heavy damage to bosses and also has piercing. Cannot be recharged by any means, not even through using lil’ batteries or charge-related items, thus becoming useless after the 6th use.

Tooltip: ‘I can see clearly now’
Type: Passive
Effect: Covers the player in several eyes. Tears up significantly and can bypass the tear softcap, but every ‘other’ shot is launched in a random direction.
Unlock: Pick up 3 eye-related items in a single run. This includes items like Haemolacria, Polyphemus, Proptosis, Inner Eye, Cain’s Other Eye, Cursed Eye, 20/20, etc.
Name: Zero-Gravity
Tooltip: ‘Zero-gravity tears’
Type: Passive
Effect: Tears down. Tears are larger but decelerate as they travel. Once they stop they launch a smaller shot forward, 2 at 45-degree angles of the first shot, 2 more 90-degrees of the first shot, 2 more 135-degrees of the first shot, and finally 1 last shot behind it, before the main tear disappears. Ultimately this means shots are launched by the larger tears in the cardinal and ordinal directions before it vanishes. The large shot can be made to disappear prematurely if an enemy runs into it.
