Video here is a sample of one way to bypass Granny Rags in the sewers + The Flooded District without failing ghost nor clean hands achievement. Hope this helps one, or two.

Still havent gotten to remember locations of hostiles and took about an hour but I managed to get through it without having to load back to check points.
In the Video;
– Granny Rags appears in the sewers since I helped her on Mission… I think it was 2, 3 guys banging on her door to rob her or whatever.
– Runes and Charms are collected since the heart gives you locations of them.
– Ghost as never detected, Clean Hands as killed no one, Mostly Flesh and Steel as I’ve purchased no upgrade/ability. I actually went through the whole game without making any kind of purchase since I didn’t find the need to.
