Overview
This guide will help you with patching the original Fallout to not only work with Windows 10 (or 7, 8.1), but also remove the censorship and allow the game to run in 1080p with hi-resolution artwork!I am the writer of the guide, but not the creator of the mod.
BETTER OPTION – FALLOUT ET TU
The original guide still exists below, but the best way to play Fallout 1 is now through the Fallout 1 in 2 (Fallout et tu) mod. You must own both Fallout 1 and 2, but it essentially allows you to play the game in 2’s engine, with all the enhancements that come with that.

Introduction
Before I even start, I want to say one thing: this game was censored back in 1997 by Interplay, not by Bethesda, due to the whole “DOOM makes people into mass murderers” farce (no disrespect to anybody who has lost a child or family member to a mass murderer/school shooter… or any murderer in general). So you can thank them.
Also: read this whole post before doing anything so there are no dumb questions.
Backup Your Stuff and Disclaimer
You may want to back up your game files (copy contents of C:Program Files (x86)SteamsteamappscommonFallout and place them somewhere else, like a folder called “Backup”), though with Steam file integrity check, that’s not totally necessary. I am not guaranteeing this will work for everyone; however, it works for me.
I am not the creator of the mod, just the creator of this guide, so issues with the mod should be directed to the Fallout Fixt Wiki.
[link]Downloading the Patch/Mod
You will be downloading the “Fallout Fixt” community patch/mod.
Go to the following website and read through the post. You will find several download links on it. I suggest downloading the “FULL CUSTOM” Fallout Fixt mod. You will have the option in the installer to install only the bug fixes/censorship removal [Purist patch], but if you want, you can have any level of non-canon stuff added (stuff that was never finished before launch, but has been finished by the Fallout mod community).

[link]Installing the Patch/Mod
Run the executable, follow your install path for Fallout (C:Program Files (x86)SteamsteamappscommonFallout or similar) and install the patch. During installation you will be asked to choose what you want to install. I would suggest going with some of the .PDFs it suggests, such as the Keyboard reference and the Fallout Bible (though this is really only for people who want to learn about the now non-canon background of Fallout and has spoilers in it!).
IMPORTANT!
After installation you will have a new folder within the Fallout folder called “Fallout Fixt.” The file within called “FALLOUTW.exe” is what allows you to play the fixed game. You can add it to your Steam library as a non-Steam game if you wish, or just create a shortcut to it (there already is one in the folder called “Play Fallout Fixt”) to put on your desktop.
Resolution Fix (optional)
The game by default will run at 800×600; however, there is also another executable given to you by Fallout Fixt (in your Fallout folder, not Fallout Fixt folder!) called “Fallout 1 High Resolution Patch (version number)”. Run that if you want to run in a higher resolution (duh). Then go into the Fallout Fixt folder and run “f1_res_config.exe”. Choose your resolution, enable the hi-res art (if you want) and run the program. Now when you play Fallout Fixt it will run in your chosen resolution. Going in-game and into Settings and clicking “Screen settings” will allow you to turn on 32bit colors, and then going in to “Game settings” will allow you to change your mouse sensitivity (because it’s low for the higher resolution).

If you need more help, like this didn’t work, make sure to read the file “Troubleshooting Fallout Fixt” that came with the download of the patch.
