Prologue
On Daring difficulty, you prevented the Chaos Spawn from consuming cultists at the end of the prologue.
Kill the cultists first as fast as possible, then kill the Chaos Spawn, and you should be good to go.
You completed the Prologue.
You will get this automatically after finishing the prologue.
Chapter 1
You killed the deamon of Tzeentch in the sewers under the star thoroughfare.
On Rykard Minoris, go to the Uppway. From there, go into the warehouse and venture into the sewers. There you will find a man lying on the ground with a Lore (Warp) check. If you interact with him, the Tzeentch Demon will spawn. Kill him and you'll get the achievement.
Playing on Daring, you did not let any shuttles be destoryed at the end of chapter 1.
As it states in the achievement, you should be on Daring difficulty (or higher) to get this one. At the final battle with Aurora, you need to kill the boss asap, or he'll start to destroy the shuttles.
You completed Chapter 1.
You will get this automatically after finishing Chapter 1.
DLC: Void Shadows
You can do those achievements in Act II as well as in Act I, but as far as I know, not later.You decided the fate if the freight line of your ship.
If you go down to the Freight Line and talk to Eddock Frensoe, tell him that you want to check out the situation, and you should get the quest The Lowest Walk of Life. Follow this quest, and at the end, you have to report back to Eddock Frensoe (You can do this most easily after finishing in the Temple of the Bloodspun Cult, when you are ready to leave. There, the NPC Eddock Frensoe can take you if you tell him that you still have business with him). Before you talk to Eddock Frensoe, it’s best to save. I chose the Iconoclast option and got the achievement. I think the other two options should also work.
You were less precise with heavy machinary.
In the Freight Line, you can lift a container with a crane arm. If you drop it on the guy standing on the yellow-and-black area, you will kill him and get the achievement.
You successfulkly mingled with people of much lower rank.
To achieve this you have to o the following:
Interact with the flute player (he is next to the burned guy and the enforcers): Play fair -> play cheat -> give button -> leave
Talk to the foreman: eat slop -> pass the skill check(Currently it's not clear if the skill check needs to be passed, but if you have trouble you can go into options > "Skill test difficulty modifier" to 75% to be safe)

You became a spinner of the bloodspun web.
You have to pass the trials you encounter on the way through the Freight Line. You need to kill the burned man, and tell the man standing on the track who wants to jump down that you didn’t come to kill him and then walk away (if you did it correctly, he will despair and jump down himself). I simply jumped into the fan using the Athletics check. When you are in the temple and the Vizier tells you that you have passed all the trials, just accept it and say that you want to join the Bloodspun Web. At the end, the ceremony takes place and you are bathed in blood (if you pray to the statue or look up at him, he will heal you from the "Suspicious Rash"). After the conversation with the Vizier, you should get the achievement.
Chapter 2 - Part I
Be sure to make a hard save on Footfall before you go to the pirate's funeral.
You stepped willingly into sewage on Footfall.
For this, you must come to Footfall incognito. Then go to the funeral and pose as Fidelio. To the left of the Book of Remembrance is a corridor with a trap. Disarm it, go to the door, and examine it. The clerk should then speak to you and tell you to go into the furnace to help him. Do this, and the furnace will be activated. Destroy the floor there and escape from this death trap. You will then enter the old tunnels and be chased by people. On the way out, you must decide who from the party goes through the sewage. Go in yourself, and you will get the achievement.
You gave the inheritance to the girl at the old pirate's funeral.
You don’t necessarily have to come to Footfall incognito. At the funeral, say that you belong to Valencius, and you can look around during the ceremony. Talk to everyone, especially Adelia (she is near the chaplain behind a crate with sheets). Once you’ve explored enough, you can go to the chaplain and review the clues with him to figure out who Fidelio might be. Repentance is the ship, and there is mention of dead eyes. After you’ve gone through everything, the chaplain will tell you that you are now on your own. Fidelio is the servitor next to First Mate Daggen. This part is optional. You can also choose not to find out who Fidelio is and decide what should happen with the inheritance. For the achievement, you must give the inheritance to Adelia (Iconoclast: Follower).
If you previously said that you are Fidelio. After escaping from the death trap return to the funeral and pick the option "I'm not in a killing mood" and give the inheritance to Adelia (thanks to 'Commit Toaster Bath' for he info)
You attempted to reach the Imperium.
This one can be done as soon as you have access to the star map. Below the Furibunds system is the Imperium. Save the game beforehand. Try to reach it, and in the following dialogues, continue through the warp without turning back. You and your crew will die, and the achievement will be yours.
You won your first void combat.
You will get this automatically after finishing your first Void combat in Chapter II.
You discovered the truth about Nomos.
To get this achievement, you have to start your progress in Chapter II and finally unlock it in Chapter IV. Very early on, you need to retrieve the hidden caches that Theodora left. One cache is on Kiava Gamma and the other on Dargonus. Be sure to explore both planets thoroughly, and you’ll find them.
Then go back to your ship’s computer and hand in the caches. After that, continue through Chapter II, and Pasqal will approach you and tell you something about the machine spirit. Help him with the ritual and let him research it. Do not kill or purge it. Be sure to treat Nomos nicely throughout all of this.
Complete all quests related to Nomos and talk to him whenever you can (for example, on Dargonus during your ceremony).
In Chapter IV, you will be informed of a tech priest who snuck onto your ship. Speak with the tech priest and do not kill him. After that interaction, talk with Nomos, exhaust his dialogue, and the achievement should unlock.
You resolved the crisis on Janus.
You get this automatically by completing the main story on Janus.
You decided the fate of Kavia Gamma.
You get this automatically by completing the main story on Kavia Gamma.
You secured Dragonus.
You get this automatically by completing the main story on Dragonus.
You drove Marazhai mad at the governor's camp.
In the Governor’s Lager on Grantis in the Platium Stellos system, where you meet Marazhai and can speak with him, you must pass three hidden Coercion checks
1. Coercion: "My, my, this Beast can speak our language!",
2. Coercion: "Aggravating, is it not - having someone best you at your own game?",
3. I don't know if it matters but I choose "We're smater than we look.",
4. after that you have again multiple Answers you can choose from and again, if you succeded the Coercion test choose:,
5. Coercion: "Hmm? Oh, you're finished pontificating? I think I dozed off there for a second - do you mind repeating the whole thing from the start?",
After the third Coercion option, he gets angry, attacks you, and runs away. After the dialogue, you should get the achievement.
You subdued the Pit.
You must be a Psyker to get this achievement. In the Speculo system on the Jungle World, you can travel to the Bottomless Pit. At the end of the bridge, there is a point of interest. To access more options, you need to pass the Investigation check on the inscription on the wall near the pit. After that, go back to the bridge, choose the option "Try to dominate the entity," and pass the Willpower check. After succeeding, you will get the achievement.
You investigated the secret bunker in the distress signal zone.
System: Cradle of Khepri. On the ice planet, you must destroy the transmitter (Iconoclast option). After that, you need to pass an Awareness check, then go to the ice wall and pass a secret Awareness check to blow up the ice wall. Soft reset if needed. Once you are in the bunker, you will get the achievement.
Note: If the transmitter was not destroyed in Act II, you can still do it in Act IV.
You solved all the puzzles at the unidentified ruins.
1. Uncover the symbol (star symbol near the rubble) and replicate it with the pillars.
2. Go to the chest in the statue’s mouth and retrieve the Ancient Force Sword.
3. Interact with the empty jaws, and your character will automatically place the contents of the chest (from the hidden chest) into the mouth. Important: The game may sometimes send the box to the cargo instead.
4. Replicate the pattern shown in the image and loot the chest (Chains of Domination, Foulsight Veil).
5. Win the fight.
Chapter 2 - Part II
You queued patiently with the commoners in Jae's second quest.
This is part of Jae’s companion quest. Obtain the required seals and return to the Administration on Dargonus. After you enter the waiting room, you don’t have to press anything else for the achievement except wait. Just keep waiting. You have to wait a very long time. This is too real. Crying in German bureaucracy.
You reclaimed all the colonies of the von Valancius dynasty.
To get this, you need to reclaim Foulstone, Janus, Kiava Gamma, Dargonus, and Vheabos VI. As far as I know, you have to visit Vheabos VI after Dargonus for everything to work.
You completed Chapter 2.
You will get this automatically after finishing Chapter 2.
DLC: Void Shadows
You were self-sufficient when dealing with the blood bath mystery.
After the bloodbath in your private chambers, you must investigate it yourself instead of letting the others examine it, and complete the rest of the storyline on your own as much as possible.
I found this guide on reddit being very helpful.
You destroyed 4 ships using attack crafts.
Destroy four ships in space combat using the attack crafts (the ones you can deploy instead of torpedoes).
You uncovered all the secrets of the voidship shrine.
Retrieve the hammer using the pressure plate puzzle and complete the two quests from the people standing at the outer side of the map (The Cube and The Confessions).
You saved all Astropaths during the telepathic ritual.
Complete the ritual with all Astropaths remaining alive. It will not work if Weisz does not receive help! I did it with the psyker from Winterscale.
You performed the telepathic ritual with Idira as your assistant.
For this, you must tell Weisz that you want Idira to assist with the ritual. Idira will not be available for the subsequent fight, which makes it a bit harder. After the fight, you will get the achievement.
You defeated the ship's confessor without letting the Rogue Trader suffer any damage.
As the achievement states, you need to finish the fight without taking damage. The good thing is that only the Rogue Trader needs to come out undamaged.
It is important to note that even taking 0 damage or dodging will prevent you from unlocking the achievement(Thanks to rh for informing me about this). The rest of your party can take damage.
You forced the Magus to kill himself with his own attack.
During Chapter II, there will be a boss fight in the crypt. He has a mechanic where a red circle appears under one of your companions or your character. A turn later, he will drop an AoE attack that damages everyone in the circle. Move your companions and your character as close to the Magus as possible and let him kill himself with this attack.
DLC: Lex Imperialis
You collected 4 different familiars at the same time.
Have four Archetyp Overseers in your party, each with different companions (one must be a Psyker/Unsanctioned Psyker for the Raven, and one must have the Arbiter background for the Mastiff—though I think Solomon also works).
On Daring difficulty or above, you "accidentally" knocked out a companion in the Warp Relay area.
Use an Archetyp Overseer as a Psyker (or Unsanctioned Psyker) to kill an ally with the Raven using Warp Relay? (on Daring difficulty or higher)
I got the achievement after one of my companions was killed by fire within an enemy zone. This is probably not exactly what was intended. I imagine it might be a bug, and the intended requirement could be that a companion has to die from a psychic phenomenon.
On Daring difficulty or above, you had at least 15 creatures at once in the Extrapolation area.
Fairly straightforward. The easiest way is to use Cassia’s Point of Curiosity ability to gather many enemies in one spot, then use Bring It Down! to give the character who has the Servo-Skull an extra turn. Then simply use Relocation to move the Servo-Skull into the group of at least 15 enemies, and the achievement should unlock
On Daring difficulty or above, you used Obstruct Vision to kill at least 3 enemies in one fight.
Have an Archetyp Overseer use the Eagle and kill three enemies whose sight is obstructed.
Important: the Obscured effect only lasts while the Eagle is present. After its turn, it flies away and the enemies lose the effect. So it’s best to bring the enemies as low as possible with the Eagle first, then have the Overseer finish them off—repeat for three enemies in a single fight.
On Daring difficulty or above, the Cyber-Mastiff bit at least 7 different enemies in one turn.
Have an Archetyp Overseer use the Mastiff (or do it with Solomon’s Mastiff) and bite 7 enemies in a single turn.
This can be done very effectively using the Protect ability.
Chapter 2 - Part III
You boarded the Heartless without destroying any of its compartments.
After the space battle with the Heartless, Ravor will suggest attacking the Heartless a bit more. Decline and go directly aboard with Solomorne. After you defeat the captain, you will get the achievement.
You embraced pain and activated a xeno-artefact on the Heartless.
After the first fight connect the power, where the Xenos apparatus is located. You must pass the Lore [Xenos] check at the console to activate it.
[Touch the control panel] -> [Toughness Check] [View the logs] -> Look through all the logs -> [Lore [Xenos]] [Open the spherical prison] -> Achievement unlock.
You defeated the captain of the Heartless.
On the Heartless, defeat the captain on the bridge, and you will get the achievement after the fight.
You successfully petted Glaito.
Pet Glaito. After the attack on the Heartless, you can talk to Solomorne and choose Hello Glaito, then try to pet him. You must pass a Lore [Imperium] check for the petting to succeed; if it fails, Glaito will bite you. You have two attempts before Glaito refuses to be petted, so it’s best to save beforehand. If you succeed and pet him, you will get the achievement.
You successfully ate all three dishes on Thassera.
After the fight with the mutants, you must return to the Governor and speak with him. He will tell you about the food available. You need to pass all the checks and eat all three dishes to get the achievement.
Note: The checks become progressively harder. I recommend starting with the skill check you are worst at. After successfully eating all three dishes, continue the conversation, and you should get the achievement.
You assembled all the evidence to expose the conspiracy.
During your visit to Tharessa, you need to pass some checks and talk to several people. Below are some pictures I took of the clues I found:
- Upon arrival on Tharessa: the first Awareness Check
- then you can talk to the govenor and point some things out like the lacerux
You found all the impostors.
The governor will invite you to take part in the imposter hunt after the dinner. The people you need to talk to are: Edytka, Man with Siscours, Old & Young Priest, and the Young Woman in the fine dress.
I chose the following and got the achievement:
- Edytka: I let her leave, but I don’t think it matters if you brand her or not
- Man with Siscours: Branded
- Old Pries: Branded
- Young Woman in fine Dress: Branded
Chapter 2 - Part IV
DLC: The Infinite Museion
You found the secret study of Seberian von Valencius
In Act II after some void jumps your crew will tell you that they found a hidden room from someone named Seberian von Valencius behind the Augementation chair. Go there and after you put you hand into the scanner to open the door you should get the achievement.
You earned the respect of both Vedka and Izdrig at the same time.
On Zarkhov do the following to be sure to get this:
- In the camp, talk with Batoshski and drink the stuff against poison.
- Go around the camp and save the dying man near Chirurgeon Lukha
- Check out the shooting range and succed to win over some of the Merovians
- In the cage resolve the conflict without anyone dying
You've done everything correct when you descend to the Drover and Vedka and Seagant Janur follow you. That's when I got the achievement.
On Daring difficulty or above, you killed the Drover without destroying its shoots.
Be sure to be on Daring difficulty or higher. In the Drover fight, just focus on the drover. If you kill any of the other plants the achievement will not pop. It's also worth noting that this is a damage race. The Growth's will eventually kill themselves if they have enough Maturation stacks(they need 3). If they kill themselves you will also be locked out of the achievement.
There is a small passage that allows you to get right in front of the boss shown in the picture.
You used every augementation slot on your character
Pretty self explanatory. Install an augment in ever slot of your character, close the menu and the achievement should unlock.
Chapter 3
You returned the boots in Chapter 3.
After you wake up in the Dark City, the Ugly Beggar will be nearby. Confront him or ask for help. He will attack you, and you will eventually kill him and can loot his shoes.
You removed all debuffs from the Rogue Trader before meeting Malice.
After you wake up, save your game first. Where you wake up, there are several Awareness checks. If you pass them, you can loot, and some contain Stimm. You need a few of these to remove all the debuffs. Additionally, some beggars also carry Stimm, so you can kill them to obtain more.
There are four debuffs: Hamstrung, Darkened Eye, Deformed Bones, and A Hundred Wounds. You also have a trauma, which you can remove with a Skin Patch. Three of the four debuffs, except A Hundred Wounds, can be removed using Stimms. After doing this, the achievement should unlock for you.
You let the Khymerae tear apart all the gladiators in the first battle in the Commorragh arena.
In the first arena fight, simply let all the NPCs kill the Wild Khymeras.
Also, it’s best not to make any speeches or similar actions—just enter the ring.
You discovered the truth about Theodora's death.
Be sure you have Argenta in your party throughout Chapter III. Get her to join your team again in the Dark City. Just before the second arena fight, she will speak with you. To get the achievement, listen to her Confession.
You obtained Sweet Perdition.
After the second arena fight, just before you go to the Homunculi there will be a story event. This achievement seems very inconsistent but here are the choices that worked for me:
- But I still decide to wait and get a better look at the Prey
- I act!
- is trying to locate me among the tangle of spiked minarets. (Awareness test succeeded)
- breaks into a run
- slips away using the bodies of random passersby as cover (Agility test succeeded)
- the hunt continues!
- (Willpower test succeeded)
- slips away with the skill of one who has lived here their entire life. (Lore (Xenos) test succeeded)
- The conclusion is drawing closer!
- Carouse test succeeded
- looks defenceless and I attack, failing to notice the trap it has laid out for me. (Demolition test succeded)
- Everything will be over in moments.
- banks on brute force, trying to break my fragile bones and shatter my joints. (Strength test succeeded)
- (Items received: Sweet Perdition)
- The pray grabs me by the throat, lifts me into the air and snarls. It seems to want to know why I attempted to kill it.
- I die.
- The chaotic burst of violence that bloodied the streets of Commorragh ends just as abruptly as it erupted. Such is life in the spires.
You completed Chapter 3.
You will get this automatically after finishing the Chapter 3.
Chapter 4 - Part I
You handed over Idira, Yrliet, and Marazhai to the Inquisition or executed them yourself.
If you have recruited Marazhai and still have Yrilit, Idira, and Heinrix in your party, you can go directly to Heinrix at the start of Act IV, hand all three over to him, and get the achievement. (But it’s best to save beforehand, unless you’re okay with giving them up as companions :D)
You completed Jae's second quest without being spotted.
In Jae’s second quest, you will have to navigate the sewer without being seen. It’s best to save beforehand and activate all the valves. Here is a map with all the locations:
You completed a colony project of the highest tier.
You need to complete one Tier V colony project to get this achievement.
You completed the highest-tier colony project on each of your colonies.
You need to complete one Tier V colony project for all your colonies (Foulstone, Kiava Gamma, Dargonus, Vheabos IV, Janus) to get this achievement.
You found a gold xeno-idol.
- Rumor: Rare Find, System: Cradle of Khepri, on the Rocky World
You wore the Halo Device.
In the Mundus Nullius system on the Ice World, there is an optional landing site called the Abandoned Palace. There is a corpse that triggers a boss fight. After defeating the boss, you can loot his body and get the Halo Device. Do not equip it right away. If you have Heinrix, Yrliet, or Marazhai in your party, they will prevent you from putting it on.
Go to your ship and into the Lord Captain’s chambers, and save your game just in case. Then equip the Halo Device. You will likely need to make some saving throws, and after that, the achievement should unlock.
You melted the ice planet.
First, you need to equip your ship with a good Auspex module (I used the X-470 Ultimo Array, granting 90% hit chance and 20% critical). Then, at the Hundred Dreams Crossroads, scan the Ice World and you will receive a contract named "Melting the Ice." You will need 1 Flogiston, 5 Promethium, and 3 Chemicals. Complete this contract, and you will get the achievement. Afterward, you can interact with the planet again and either loot it or report back to the Mechanicus about it.
You killed a defiler without destroying any of the 4 plasma batteries strengthening it.
Defeat the Incongruous Defiler on Euphrates II without destroying any of the four plasma batteries. Pretty straightforward.
You surrendered Eufrates II to Chaos.
Give yourself to Chaos. When talking to Uralon, choose the "Sameth" prompt. Pledge your allegiance to the Edge of Daybreak, and you should get the achievement.
You recruited a secret companion.
There are a total of three secret companions you can recruit: Calligos Winterscale, Chorda Incendia, and Uralon the Cruel.
Here’s how to recruit the secret companions as far as I know:
- Calligos Wintersacle:
Be on his site and help him on Quetza Temer. You need to kill the Asuriyani and gain his favor. After the events on Quetza Temer, and before transitioning into Act V, he will come to your ship and ask to accompany you. Agree, and he will become your companion.
- Chorda Incendia:
On Footfall, you will meet Chorda again. To recruit her, be on her side and choose the dogmatic options, including fighting and killing Vladaym Tocara.
There might be a way to recruit her using Iconoclast options, but I’m not sure about that route.
- Uralon the Cruel:
Recruiting him is straightforward if you follow the heretic route. After the fight on Euphratis II and battling the Space Wolves, he will come to your ship before you transition into Act V.
You completed Chapter 4.
You will get this automatically after finishing Chapter 4.
DLC: Void Shadows
You took a bath with every available companion.
Once you are back on your ship and have recruited Marazhai and Ulfar, you can earn this achievement (all main companions required).
You can complete this as soon as you reach Chapter IV and have recruited Ulfar and Marazhai.
For this achievement, you must take a bath with all companions (except Solomorne and any future companions from DLCs), including: Abelard, Idira, Argenta, Cassia, Kibellah, Pasqal, Heinrix, Jae, Yrliet, Ulfar, and Marazhai.
You saved Captain Klein on the freight line.
During Chapter IV, there will be an event on your ship where you are trying to regain control after a mutant attack. Eventually, you encounter a fight between some mutants and your soldiers on the train rails. You will have the option to navigate the train to kill some mutants and some of your soldiers, or choose to leave the console and not kill anyone. Choose to leave the console and rush in to save your people. After the fight, you will speak with Klein and get the achievement.
VERY IMPORTANT: Jinevra must NOT be killed during the investigation in Act II! If she is killed, Klein will NOT appear in Act IV at the Freight Line, and you won't get the achievement.
Chapter 4 - Part II
You were a little too easy to convince.
In Act IV, at some point between the warp jumps (for me it was after visiting Footfall), an event occurs where the entire ship is attacked and you crash in an elevator. At the very end, you return to the bridge and can speak with “Sire” (a copy that looks like you). In the conversation, simply choose:
- "What are you"
- "If you are a sentient creature, then you are also capable of negotiating, are you not"
- [Give up the futile struggle] "You know... that unity of yours does not sound so bad after all. I think I am ready to accept it."
This will unlock the achievement. Attention: This will give you a game over!
You defeated the Genestealer Cult on your voidship.
Similar to Fool Me Once, but you fight the Gen Cultists, and at the end of the dialogue chain, you will get the achievement.
You defeated the vision of the Undying One.
Finish Kibellah’s storyline and defeat the Vizier.
You dispelled the shadows in the void.
Complete Kibellah’s entire companion quest and romance her (I’m not sure if the romance is required). I followed a guide for the choices, selected the Synchronicity options as well as the Humility options, and got the Humility Ending. After the fight with the Vizier, I received the achievement. I’m not sure if other endings also unlock the achievement, but this worked for me.

DLC: Lex Imperialis
You personally got into the tank.
In the first fight on Leethus, after killing a few enemies, a dialogue will appear where Solomorne suggests that someone must enter the tank. Do it yourself, and the achievement is yours.
You hit at least 7 enemies with a single shot from a tank.
Kill 7 enemies with a single shot from the tank. The easiest way is to use Cassia’s Point of Curiosity ability to gather the enemies in one spot, then reload the tank with her so it’s ready to fire. Shoot into the group of enemies, and the achievement should unlock.
You knocked out your companion with a tank shot.
This works well in combination with Tank You. Just shoot an ally with the tank. The achievement also counts for companions, like Glaito. (Not that anyone would actually do that…)
You received a complete inventory of the Rogue Trader.
When you first talk to Clerk Tertius Quart on Leethus, simply answer all his questions and let him document your retinue once. He will then want to document you as well. Let him do it, turn around for him, and undress for him. Do this, and you will receive an item along with the achievement.
You poked your nose into all the available containers on Leethus.
For the achievement, simply loot everything available on Leethus. Interact with all objects you can—sometimes this will unlock additional loot. Also, interact with things that Glaito can do (for example, crawling through the sand tube to retrieve a chest). After looting the area of the Sand Boss, make sure you’ve checked all containers. That’s when I got the achievement.
You discovered the identity of the watcher of Leethus.
After all the chaos on Leethus, you arrive at the Watcher’s station. There, you speak with the Watcher and, after a while, ask him who he really is. You have three skill checks available. To get the achievement, I had to choose the Commerce option and then claim the Tithe for myself. When Solomorne commented on it, I selected the Dogmatic option. Only then did the achievement appear.
As far as I know, the Iconoclast and Heretic options to persuade Solomorne should also work if they are available to you.
The Logic and Tech-Use options did not work for learning his name.
You answered all accusations in the Palace of Justice.
During the trial against you, simply respond successfully to each accusation using one of the skill check options, and you will get the achievement. There is an autosave before the trial begins, so if you fail one, you can just reload and try again or make a hard save beforehand.
You experienced the full force of the Lex Imperialis and survived.
You will get the achievement at the end of the DLC, after you determine the fate of the Marshal.
DLC: The Infinite Museion
You completed Zarkov's colony chronicle.
For this achievement you need to finish a Rank V colony project for Zarkov.
On Daring difficulty or above, you destroyed the Aeldari ship in Zarkov's orbit.
After you get the distress signal from Zarkov that Aeldari ships appeared. I got it after I destroyed the Wrath of Khaine ship.
Chapter 4 - Part III
On Daring difficulty or above, you saved everyone in the settlement during the Flayer's attack.
After the Void Battle with the Aeldari above Zarkov you can fly to the camp. There the settlement in getting attacked by Flayers. Make sure that no NPC dies and you will get the achievement. Be aware of the blood damage they inflict. Heal the NPCs as needed. It does not prohibit you from getting the achievement, if people from your retinue die in battle.
On daring difficulty or above, you broke the Chronomancer's obelisk before it's first activation.
After descending to the tomb, you encounter a Chronomancer in battle. There are plenty of obelisks in the arena. For the achievement you need to destroy only one before the Chronomancer gets his turn. With an Officer + Grand Strategist this is manageable because as a Grand Strategist you move first and with the officer abilities you can give another member an extra turn to help you destroy an obelisk. But be aware that you only have 30 seconds on extra turns because of the Chronomancer's passive ability.
You tried to approach the Chronomancer 3 times in 1 round.
After descending to the tomb you encounter a Chronomancer in battle. The Chronomancer has a passive ability called "Temporal Regression" that sends you back to the location you started the round, when you approach him. Do that with three characters in one turn and you should get the achievement.
I had to do it 4 times in one round, because one of the times I did it with the charge ability and that might not count towards the achievement but the ability still resets you position. So to be sure just walk up to the Chronomancer with multiple characters in one turn and you should be good.
You solved the puzzle in the lost tomb on Zarkov.
After the fight against the Chronomancer you can explore a bit and when you clear the path you find Chest. To open it you have to get the right combination of the console next to it. The combination is: Right lever, Middle Console, Left Lever, Left Lever
You activated the xenodevice in the hardlight lab.
After the fight against the Chronomancer there is a door you can open after examining the tech priest corpse. You are now entering the Museion. Talk with the Astartes in the middle of the room. Then go to the console and choose the option "Activate the rightmost chain". After that you should get the achievement.
On Daring difficulty or above, you took no damage from gas when fighting the soldiers of Krieg.
In the Museion you'll eventually encounter another Chronomancer whom you can talk to. After interacting with him you have to fight Soldiers of Krieg. There are two masks you can equip, preventing the two characters who get them from taking damage from the gas grenades the Soldiers throw at you. I recommend having two characters do the killing and to put the rest of them in the back to make sure they don't get targeted by gas grenades.
After the fight is over and you didn't take gas damage you'll get the achievement.
You solved the puzzle in the Museion.
Here you'll find tiles on the floor where you need to connect the energy from the outer points to the two middle ones. After you managed to do this you'll get the achievement.
You looked at all the exhibits in the Museion.
When entering the Museion be very thorough. Look at everything where there is a magnifying glass symbol. Also talk with Trazyn every chance you get, so he can explain some of the exhibits you can't examine. If you looked at most of the stuff before returning to your ship you should get the achievement. In the picture I marked all the locations where you can talk with Trazyn.
You handed Seberian over to the Inquisition in the time loop.
After the Museion you enter the Dragonus Exhibit. There you'll get stuck in a time loop where Seberian dies and then the siege on Dragonus repeats itself. For this achievement you have to cooperate with his daughter Juvenna von Valencius. You have to follow her plan to get Seberian to talk to the Inquisition. Follow the steps for the Quest "Whispered Negotiations".

If you have done everything correct you have to battle the Inquisition and after that you'll get the achievement.
You did not deviate in your management of Sector Bionica.
For this you either have to always side with the Vivisector and allow everybody to get implants or always side with the Genetor and reserve the implants for special people. After the Sector Bionica got attacked, soon a procession will be on the way to the Voidship Bridge and after that event the achievement unlocks if you stayed true to one side.
You solved the puzzle on Tirkuaz-4.
On Tirkuaz-4 on the top left there are some valves you need to interact with. To Solve it you can simply interact with the right valve two times and the light should turn green and the achievement should unlock.
You upgraded the Implantation Throne in the Sector Bionica to the maximum level.
Eventually you will get Implants you can't equip in the Sector Bionica. After I interacted with the Augmentation Throne I got the Quest "Premium Implants". Go to Altar-Templum-Calixis-Ext-17 and there they'll tell you about missing shipments of promethium on Tirkuaz-4. Complete "Industrial Sabotage" and the achievement will unlock.
Chapter 5 - Part I
You learned the necron language.
In the first system in Chapter 5, on the planet located at the top-right (Debris of Battiada: Ice World, Signal from the Planet—behind a Tech-Use Check), there are three checks you must pass on the planet (2× Lore [Xenos], 1× Tech-Use) before interacting with the cogitator in the center. Once you’ve passed all the checks, interact with the cogitator, pass the Tech-Use Check to [Examine the machine] -> [Confirm activation of the information panels] -> [Initiate final cataloguing] -> [Transfer the data to your Elucidator] -> [Try to extract the data concerning the mechanism's masters] -> [Continue the attempt] and you will get the achievement.
You destroyed at least one ship of every enemy fleet (pirates, Chaos, Asuryani, Drukhari, Necrons).
Here I have gathered some intel in my playthroughs and documented a lot of space battles and what ships I encountered. Some of them are labeled iconoclast ships, but I don't know what they count toward, so if you have any information, please let me know.

In Chapter 5, you have the opportunity to fight Necron ships. After destroying one of them and defeating all other enemy fleets, you should get the achievement.
You visited all the systems in the Koronus Expanse.
To get this achievement, make sure to visit every system in every chapter as you progress through the game. As far as I know, systems tied to companion quests do not count toward the achievement, but I’m not 100% certain.
If you have visited all required systems, the achievement should unlock in Chapter V after visiting the last one.
You scanned all worlds and reported them to the Explorators.
Yeah, so this achievement is about as buggy and inconsistent as it gets. You would think scanning all planets and handing everything in would do the trick, but that is not the case. After four complete playthroughs, I haven’t managed to get this one, and in the last two I was very thorough. I’ve read a lot of people having similar issues with it.
I recently found this Reddit post that you may want to try. I haven’t tried it myself yet, but maybe it helps you a little.
I also found out that the Tech-Priest, to whom you can submit information about scanned planets, has a range check. In Chapter 2, the maximum range is about 110, but in Chapter 4, the maximum at once is around 70. This means that if you are beyond this range, you cannot submit information to him, and the dialogue option will be greyed out. The counter for scanned planets decreases every time you submit them to the Tech-Priest. This explains why many players couldn’t submit information in Chapter 4 if they hadn’t submitted anything earlier, because the amount of scanned planets was above the range limit and effectively blocked the option.
And if nothing else works, you can always install ToyBox and unlock this achievement manually.
You raised your own abomination.
This achievement is tied to The Possessed Ship. At the end of the game, Nomos will ask to inhabit the body of the tech priest from Chapter IV. Accept his proposal to join you before entering the last planet, and when deciding what to do with theC’tan Shard, let Nomos absorb it. The achievement will then be yours.
You became a votary (Dogmatic) on Daring difficulty or above.
You need to have reached Rank III in the Dogmatic Conviction by the end of the game on Daring difficulty or higher to get this achievement.

DLC: Void Shadows
You completed the game in grim darkness mode.
Complete the game in Grim Darkness mode. (Tip if you are just interested in the achievement and not the challenge: If you close the game via the Task Manager, your progress will not be saved, and you will have to restart from the last save.)
You caught a suspicious rash on the freight line and did not cure it by the end of the game.
During your first visit to the Freight Line, at the very end near the large fan, you will eventually get the Suspicious Rash debuff. Simply do not heal it and finish the game with the debuff.
Chapter 5 - Part II
DLC: The Infinite Museion
On Daring difficulty or above, you smashed the ice blocks on Amsanct using only generator overload.
You can get this one when entering the Secret Facility on Amsanct. You will have to fight some Necrons and in the Facility are multiple small and big ice blocks. Every round you have to destroy specific small ice blocks so the wire reaches the big ones. In the picture I marked which ice blocks have to be destroyed in which round. The ice block with the number 1 has to be destroyed in the first round and after you destroyed it activate the console. Do the same for round two and three and you should unlock the achievement.
You decided Nuncius's fate.
For this achievement to unlock you have to finish the story of Eugunn on Amsanct.
You sent Batoshski or Dunawski on a doomed mission.
During your attack on Trazyns Museion you will have one of those Storybook moments. At one point I got the option "... the Arch-Militant and her loyal fighter from Zarkov sacrificed themselves to buy us time." I had Vedka on my ship so the exact wording might be slightly different if you have Batoshski with you. Either way choose the option to let them sacrifice themselves and you should get the achivement.
On Daring difficulty or above, you killed the Tech-Priests before they could reinforce the Defiler.
I got the achievement by killing the four Demented Heretics that are positioned around the Defiler.
Note: If you have the ToyBox Mod installed the achievement will not unlock. You have to disable the mod in order to obtain the achievement. That might be the case for other mods as well.
You gave Seberian von Valencius and Nomos to Trazyn the Infinite.
When fighting Trazyn he eventually will offer you a deal to trade Seberian for the Warrant. Accept his offer. After that he proposes another deal where he wants Nomos in exchange for his help against Calcazar. Accept that offer as well and the achievement will unlock.
You survived the machinations of Trazyn the Infinite.
When fighting Trazyn he eventually will offer you a deal to trade Seberian for the Warrant. Reject his offer and continue fighting.
After defeating him, he will open a portal for you to step in. Do not step into the portal. After your back on your voidship you have a talk with Seberian and you unlock the achievement.
You fell into the trap of Trazyn the Infinite and became a permanent exhibit in his collection.
Same as Infinite Museion but this time you do step into the portal.
This will end your playthrough! After seeing what happend you'll unlock the achievement.
You used galvanisation on augments in every slot.
For this achievement you need to use the galvanisation ability for all the main slots. You need to use them in battle but this doesn't have to be in one battle, you can do that in the course of multiple battles.
Unfortunately it seems that not every augment that can be galvanized counts towards the achievement. The ones that worked for me were the following:
- Optical Insertion: Cerebral Holo-Ocular (MK "Operative")
- Cortical Insertion: Extraction Node
- Somatic Insertion: Weapon Optimiser (MK-II)
- Sinistral Insertion: Hand of the Cursed
- Dextral Insertion: Chymist Combat Infuser
- Musculoskeletal Insertion: Grave-Core Circuit
For Dextral Insertion I tried the Hand of the Righteous but it didn't count.
For Sinistral Insertion I tried Suppression Manipulator but it didn't count.
Chapter unspecific
Here I’ve listed all achievements that are not specific to any chapter.
You killed 5 enemies with one attack.
This one is pretty straightforward. With Cassia’s Stare, it shouldn’t be too hard to kill 5 enemies with a single attack.
You restarted the schields on your flagship and received no damage to the hull while doing so.
For this, it’s best to stay far away from enemies so they can’t hit you. Once your shield has recharged and you’ve taken no damage, the achievement will unlock.
You won 10 void combats.
As the description says, you have to win 10 Void battles. There are enough in Chapter 2 to unlock this achievement.
You charted a new path through the warp from one system to another.
In Chapter 2, after visiting Footfall, you will have access to the star map. There, create a new path to a system by spending your navigator’s sight.
You visited half of the optional locations.
Go to every system, scan all planets, and visit all planets you can actually land on. I’m not sure if companion-related locations count toward this achievement.
You visited all optional locations.
Same as Curiosity Is Not a Vice, unfortunately this is also a very buggy achievement. Be very thorough. Some locations are hidden behind skill checks, so save frequently and try to pass them as needed.
You killed an enemy with 100 damage dealt by one attack.

You dodged 500 enemy attacks with your party members.
Your dodge is tied to your Agility. Having companions in your party with high Agility scores will help you get this achievement. Companions who primarily benefit from Agility are Argenta, Yrliet, and Marazhai.
You prevented 7 combats through dialogue.
During your playthrough, choose the dialogue options that prevent combat. It should be easier to unlock this during an Iconoclast playthrough, as it sometimes offers more peaceful options.
You gained a second-tier doctrine.
You will get this automatically during the game when you reach level 16 and choose your next subclass.
You gained a third-tier doctrine.
You will get this automatically during the game when you reach level 36 and choose the Exemplar subclass.
You started a romance.
Start flirting with your companions, and you should get this achievement.
Romanceable characters:

For this one, you need to pick certain dialogue options when interacting with your love interest to achieve the True Love ending. If you’ve done everything correctly, you should get the achievement when finishing the romance storyline for your love interest.

It also helps to install Toolbox to see which dialogue options will increase the flag needed for the True Love ending.
You completed 20 orders.
As the achievement states, you need to complete 20 orders. Be mindful of your resources, as you may be unable to complete certain orders if you are missing materials.
You destroyed 30 torpedoes.
This achievement needs to be farmed deliberately, as in normal space combat it’s rarely worthwhile to shoot down torpedoes. Most of the time, you just wait for them to self-destruct. The fighter squadrons introduced in DLC1 are very efficient at intercepting torpedoes, but they do not count toward the achievement.
If it’s not urgent, you can wait until later in the game when the Flak mode for the macro cannons on both sides is unlocked. This usually allows you to destroy torpedoes with a single shot. The achievement does not carry over multiple playthroughs, but you can farm it in a single playthrough by repeatedly loading a save.
The best method: save just before destroying a torpedo, then reload and destroy it again.
In the Hundred Dreams Crossroads sector, there is a fight with two Infigel-Class Raiders that works well for this.
DLC: Void Shadows
You killed 3 enemies in one turn using the Death Waltz ability.
This one is pretty straightforward. You can get it as early as the first time you encounter Kibellah by letting her kill the peasants.
Using Heroic/Desperate Action is another effective way to achieve this.
You made effective use of every special zone in void battles at least once.
In Space Battles, special zones now appear. Make sure to navigate your ship through these zones and use them to your advantage. Here’s a list of the zones and some systems where you can find them:
- Gas and Dust Cloud: Trinnitos, Silence of Mer-Segrit(some jumps after first visiting Dragonus), Hundred Dreams Crossroads, Pillars of Viridice(attacking the Heriokh's Rim Station), Porta Septentum, Cranach, -An#8954№01-KT, Sanctum of Taaphetus,
