It may seem obvious, but this guide is about achievements and achievements only.
Thus, I'll not talk about any concepts of the game outside those, which are needed for an achievement.

Lastly, let me link to a video by Thierry Henry, showing the (almost) infinite close combat combo. Almost as in as long as you have remaining chakra to keep it going. The reason I'll talk about this is simple… there are a few achievement related fights in the story, which you can't S rank without it.
So, how does it work:
If you've played the game for a bit – which you'll have until you reach the point, where you actively need this – you'll know, that you can chakra dash by pressing
Xbox – Y and A
PS4 – /\ and X
What you now need to know, is that you can actively cancel – or reset - your close combat combo by doing this during said combo. The result will look like this:

Important: in order to receive S Rank, you need to have 90% or more of your health left at the end of the battle!
Also, I've only tested it with the xbox controller, so I can't guarantee, that the PS4 inputs are correct.
Note, that you can change your active character in a fight with R3. Only if you have other team members, obviously.
Personally, I got S Rank on every fight with the original starting character and by just spamming one move over and over. Either the secret technique (
xbox - Y,Y,B
PS4 - /\, /\, O
) the normal jutsu or the close combat combo explained in the first section of this guide. I'll just add the method I used in the (I did it with) IDIW column.
Secret technique = ST / Close combat combo = CCC.
IA – Interactive Actions
Left, Right, Up, Down – Directional buttons
L, R – Shoulder buttons
Alright.
Fight 1 – Ninja History 1 – IA before & after the fight:

Phase 1 – battle phase:
Whenever he opens his mouth, just jump to the side with
Xbox – A
PS4 – X
If he's doing his full-area arm swing, you have to jump over it as well. You jump longer and higher by holding down the button.
Phase 2 – chase phase:
Kurama moves by himself, you only have to press left and right.
A red warning will appear at the upper center of the screen at the end of the chase. IMMEDIATELY move to one of the walls, or you'll receive massive damage (in S Rank terms).

Once Obito is down to his last ~1/3 of health, he will start to use the substitution jutsu really often.
Bad for you, cause he follows up with an immediate attack, which will quickly down your life under the required 90%.
The best way I found to deal with it, is to plainly wait until he attacks you, use substitution yourself and follow up with the Jutsu.

Play this fight on 30fps! With 60fps, I failed the fight over and over for more than one hour straight, because the game always decided to throw 4 substitution jutsus back to back at me in the second half of the fight. With each substitution came an instant counter, and with that damage. Enough to bring me below 90% health. Always. One-hour-straight. Couldn't do anything, because the substitution is just a guaranteed combo breaker.
But after I reduced the fps to 30, I had 2 fights, which I failed (sub 90%) within the last hits and got S rank on the third try. It may very well work for you on 60fps, but if it doesn't - try 30.

Once you entered the phase with the tailed beasts heads on her body, always look at yourself. Your body will be covered in a red aura, shortly before the heads will fire energy beams at you. Fight 2 - History's Winner - IA after the Fight

Fighting Achievements
Most of these achievements are pretty straight forward, self-explanatory and will unlock by itself, while you go for all the other achievements. So I'll keep this as short as possible.

Reduce the opponents health to 0 with the Secret Technique

You can swap the active player with R3
To do this, you'll need to fill your "storm-gauge". The yellow bar just beneath your chakra bar.
You fill the gauge by dealing damage, receiving damage, and actively using your team members.
(Yes, you actually need to receive damage to this...)
Once the gauge is full, the game will display a message on the side of your screen, that you're now able to perform a Linked Secret Technique.
Just press the displayed buttons -
xbox - YYYB
PS4 - /\/\/\O
- and your character will start glowing red, before you and your team members run towards the enemy one after another.
Notice, that this ONLY works for specific teams!

The modes you'll have to play in order to unlock the beforementioned achievements can all be found in the "Free Battle" section.
You can play each mode both in single match or team match. It is completely up to you, which one you want to play. Personally, I did it in single match, because the opponents team members interrupted me more, than my own helped me.
Though in team match, you can swap your active character during a close combat combo, which allows you to beat your opponent in one single combo. Really powerful, if you manage to pull it off.

Adventure Mode / Trail of the Gale
Trail of the Gale = Adventure Mode, as said in this sections title.
The game explains pretty much everything to you.
You can open an event overview by pressing the upper R button. Every done event has a marker.
Lines between the event icons show you, which events you need to finish, in order to unlock new ones.
In the area selection screen, which you reach via the hidden leaf gate waypoint, you'll see, if there are open activities on any of the areas.
Finish every red and blue marker on every area, and the two achievements are yours.
I'd recommend doing every green marker as well, since those count towards the 50 ninja tasks - one of the sub events - and can all be done within seconds.
Regarding the memory crystal collection sub event: You ONLY need to FIND them all.
And regarding the ninja world tournament: Rank D is sufficient for the sub event achievement.

[DLC] - Road to Boruto
S ranking in Boruto's tale is actually waaay easier than any fight in the main game, if you do the following:
Once you gain free control over Boruto in his tale, open the map, search for the shop with the shuriken icon, go there and buy both "Value Ointment" and "All-Purpose Ointment".
Now open your menu and equip both for use in battle.
These, alongside a 3rd ointment you've already equipped by default, restore health. A large amount of health.
You can easily heal yourself up from a yellow health bar back to full life. And S rank is only measured by the health you have at the time you defeat your opponent.
There is one fight in Boruto's tale, where you don't have access to the ointment.
This fight is Naruto & Sasuke vs Saruiwa. Or, alternatively labeled, Armored Kurama vs Saruiwa.
But don't worry, this fight is just as easy, once you know, that you're invincible during the complete animation of your jutsu! Makes it laughably easy.
There are 3 fights in Boruto's tale, which feature cutscenes with IA's:
First fight in the tale
Second to last fight
Final fight of the tale
Regarding the main tale Chunin exams questions: 1: What was the name of the religion that the Sage of the Six Paths taught to people long ago?
Most of the side quests will only be accessible after the main events of the tale.
Even the two, which are available earlier, can only be finished after the main events, so just do the story first.
The side quests are completely self-explanatory. Finishing all available ones, will automatically unlock new ones.
Yes, you do need to fight all of the 43 battles in the VR facility... the rank is unnecessary.
