A: No, it's the hardest to write a guide for. It's really pretty simple. Here's how it'll work. I tell you what to do and in what order, you do it, and you'll get the achievement. Almost no matter how bad you are at games, you'll get it. Don't worry. Trust me.
Q: Everyone talks about not leveling too high? Are there lines you can't get if you level too much?
A: Don't worry about it. They exist, but we don't care about them. There's like two. Forget it.
Q: I found a line not covered by the guide. Does it count?
A: I don't know. Write it down and find out. If it's not part of our 250, we don't care.
Q: There's like 8 guides for this already. What makes this one worth reading?
A: I tell you step by step what to do. No "maybe this counts", no "you should go here and just grind til you have about 80 lines", no "there are 8 lines for this one situation, but I only wrote down one of them but I'm sure you'll find the rest". Just follow the steps, and have faith.

PHASE 0: Preparation
1. Beat the game normally. No, really, don't do this and the other 5,000 things you need to do in your first play at the same time. There's NG+ only stuff and achievements you'll want. Trust me. Wait. Also the rest of the guide will have spoilers, so go away. You on NG+ now? Welcome back.
1.1 Keep a series of rolling saves going back to the first dungeon. I don't know how you could mess it up, but if you DO mess it up somehow, you want a lifeline.
2. Fuse some decent personas. You'll want one that's basically invincible, a tri-auto persona (one who you equip to start battles and has auto-matarukaja, auto-marakukaja and auto-masukukaja) and a decent high level one that has a non-instant death weakness and isn't immune/reflect/drain to physical. Like half the high level trash personas are good enough, it's not super important, it'll save you a little fuss later is all. None of these is mandatory, you can wing it. Someone with Stagnant Air might be helpful too.
3. Read the WHOLE guide first. You're tracking 250 things, you got to know what you're doing before you do it.
4. Don't level Rise's Social Link until I say so. I don't know what guides will be incompatible with my advice, but Hurricanehaon's guide will work as long as you finish the first 30 particular lines during the August 1st TV trip to Mitsuo's video game dungeon (see? I told you there'd be spoilers).
5. Bring plenty of cash. You'll need a few hundred thousand to get comfortable personas, and another fat stack to buy a ton of stupid elemental attack consumables.
6. Play through the game as you see fit until Rise joins the investigation team. Try not to be too freakishly overleveled because I don't know if that'll break anything, but it probably won't. If it does, you'll find out on line 1.
7. Learn how to Analyze churn for new lines. In short, if you meet the criteria for a line, but you don't get the message, just select analyze, pick an enemy, then back out. Did Rise say the line? If not, do it again. Repeat until you get the line. Rise will not give you the same kind of line twice in a row, but luckily "X enemies remain" is always a choice so as long as you're trying to get any other message, or if you qualify for any other message, you can always get it. It feels like there might be a maximum number of possibilities at a time, so if your whole party is poisoned and also attack down/defense down/dodge down, and all low on health, you might not be able to get all ~16 messages you technically qualify for. They also seem to roll in order of precedence, so if you have all 4 members with defense down, it'll roll MC first, then Yosuke, then Chie, then Yukiko, so assuming it's a 50/50 chance, in 100 trials, ~50 of your defense down messages will be about MC, ~25 will be about Yosuke, then ~12 for Chie and Yukiko. That means if you qualify for 12 messages (debilitate on every character, maybe), it might not be worth trying to roll no-no-no-no-no-no-no-no-no-no-no-yes to get the last one on the list. Basically, control the messages you qualify for to make it easier to roll the ones you need.
8. Don't rush during combat. Rise won't start a new line if she's in the middle of another line.

PHASE 1: Basic Tasks
0. Read this whole section. Even if you did it in phase 0, just do it again, it'll take you like one minute. If you don't, you'll finish the first 30 lines and then go "aw crap, I got half of these already but didn't know to write them down". Don't get scared, this is a lot of stuff to track at once, but you got this, and there's not much left after it.

2. Check out the passive tracking section of the spreadsheet. Track those as you do them. They include:
-defeat 1/2/3/4 enemies
-down enemies one after the other (i.e. consecutively)
-weapon critical hits
-dodges
-win fights
Also read ahead in the spreadsheet, for stuff like enemy downs (one at a time, or groups)
3. Kick things off by getting lines 1-30. 1 enemy can be found on Castle 3 or 4, 4 enemies can be found at Bath 2, 5 enemies are at Bath 6 (or later, Heaven 2), and 2 & 3 enemies can be found pretty much on any floor anywhere. Later dungeons can also work. If you don't get ANY of these lines at the start of an encounter, you're either way too strong or way too weak. You should be able to figure out which one. Once you're done with these 30, you can level up Rise's Social Link and get the rest of the lines on your own schedule. (220 lines to go. Great start, dude!)
4 Each time you find a new number of enemies, churn lines until you get all the lines that correspond to that number of enemies remaining. That'll get you another 11. (209 to go)
5. Use Rush Mode to pick up 3 more lines. (206)
6. There are 4 escape lines. One for trying to escape, the second for Rise taking a long time to find you an escape route, the third for trying to escape when the option is greyed out from a previous character trying to escape, and the fourth for actually being offered a way out. (greyed out choice may not count, so we'll say 203 to go)
Extra Credit: trying to flee from a boss you can't flee from gives a different line. Does it count? YES.
7. Down an enemy and dizzy it with a physical attack. You get a line when it gets its next turn. (202. It's progress)
8. Get yourself (MC) knocked down (whiffing a melee attack works) and then get up on your own (not be helped up). Might take a few tries. If that's too annoying, save it for when you have a baseball bat and can self-down basically at will. (201)
Extra Credit: put battle continue on in the difficult settings, let MC die and continue for +1 line. If you're severely underleveled, you can get 2 or more from just encountering enemies way higher level than you. If enemies flee during combat, you can pick up another +3 depending on how many enemies are left. Have a second group of enemies show up after you beat the first (requires there being 2 shadows close together on the dungeon map). Have characters run due to fear. Have characters get dizzy. See special lines from bosses, including optional bosses (reaper and secret NG+ boss).

PHASE 2: Freedom!
1. Play through the game as you see fit until Naoto joins your party. You probably SHOULD level Rise's Social Link to 2 or higher. In fact, yeah, go away and come back once you've done that.
2. Before heading into the TV ON A RAINY DAY, buy as many elemental attack items (both single target and all target) as you can.
3. Now get the 26 easiest lines you ever got: let Rise tell you when things are weak to an element or immune to an element when you enter combat. Update: Striptease 5F has a red shadow that consist of a killing hand and a knight. This will give you the physical weak lines, and is relatively easy to farm. (173 lines left)
4. Now to grind 24 lines by downing 2, 3 and 4 enemies with each character. You have to down the correct number of enemies without knocking them all down to get a character specific line. There also can't be a prompt for a followup attack, or you'll get the generic line. For the 4 enemies, I like to find the 4 rainy dudes and 1 samurai encounter on Lab 5 and throw the attack-all items at them since they can't miss, and you can get 2 rounds of 50 damage off from a single encounter before they die (note: kill 4 is also line, but it's extra credit). This is why it's a rainy day. Make sure you get all 7 character specific lines, as well as the generic one for each enemy count. (149 left)
Note: while you're doing this with Kanji, Teddie and Naoto, don't forget to track all their new lines!
5. Go find the Mind Dice in Lab 7 or Lab 8 and get debilitated. Get those stat down lines, my dude. Remember to NOT use your tri-auto persona, and watch out for any of your party members who have auto skills too. I try to forget them, since a tri-auto persona obsoletes them 100% of the time, but if you like them, just take your problem teammate to the mind dice alone so they won't run out of mana throwing debilitates everywhere you don't need them. 3 stats x 7 party members = 21 new lines (128 lines remain)
6. There's lines for every ailment. If a character still has an ailment on your turn, churn til you see it. For panic, MC needs to take his turn with that ailment to get the line. If you can't churn a line for MC, try getting the ailment solo (so nobody slaps him out of it) and taking a turn and see if that gets it. MC doesn't have a rage line for some reason.
6.1 For Enervate, I like the Creation Relics in Strip 1. You can also get 5 lines of "be downed by swinging and missing at enemy". Yukiko seems immune to falling down this way (due to her fan being ranged?)as well as Naoto with her gun. (121 lines to go)
6.2 For Rage, Strip 5 has a set of 2 Liberation Icons that'll valiant dance you. Note the MC does not have a rage line, at least not that I've been able to ever trigger. (115 lines to go)
6.3 For Panic, Bath 2 has a really nice encounter with 3 cupids and 1 basalt that takes single digit damage from physical attacks. The cupids only do single target panic, but there's 3 of them so it's fine. (108 to go)
6.4 Now Exhaustion in Lab 1 or Lab 2 from Anima Relics. (101 lines remain.)
6.5 Go do Poison in Lab 5 or Lab 6 from the snakes that use poison mist. (94 lines remain)
6.6 Do Fear for anyone who still needs it in Heaven 2/Heaven 3, off of Dark Eagles (87 lines remain).
Extra credit: let your party members run away due to fear.
6.7 Silence Green Sigils, Heaven 6 or 7. (80 lines remain).
7. Let everyone die but MC. (74 lines remain)
Extra credit: take some huge hits. There seems to be 1 line for being low on health (that you can churn for), and 1 line for taking a big hit for each character. There are also like 3 that don't count you hear out of combat, so ignore those.
8. Start the next fight with everyone on 1 hp, then churn for their low health lines. There's most of the party being low health too. (66)
9. If HRF still hasn't popped for you, finish off the passive half of the spreadsheet to the extent you can.

PHASE 3: Break glass in case of emergency
So you finished the passive half and you still don't have it? Don't worry, there's still a few things you can do to eke out extra points that aren't completely terrible.
1. Go to Heaven 9 and run into a group of 5 hands. Let them all run away. You should get a line for each one that flees except the last one.
2. Lure shadows near other shadows. When you defeat the first group, a second group will appear. There are 10 lines depending on the number of reinforcements, 2 lines for each possible number.
3. Big hits. There seems to be a line for each party member taking a large hit without dying (withstand is fine).
