Welcome fellow Achievement Junkie... At least that's why I'm going to assume you're here! This guide will attempt to solve any confusion you may have over any Achievements in The Binding of Isaac. Basically, it'll tell you the requirements for each Achievement!
Sure, all of this information is on the unofficial Isaac Wikia, but not everyone knows about that and being quite the contributor over there I thought it'd be worth bringing the guide over here, with a few changes here and there! :)

May 2015: Eternal Achievements!

Help! My Achievements Won't Unlock!

Never fear, for we have a guide just for you. This guide goes over the fixes available for both Windows and Mac users, so if your game isn't giving you your hard-earned achievements this is the place to look!

Progression Achievements

As I'm sure you're all aware, you can either have The Binding of Isaac (That's to say, the Base Game) or you could have bought the DLC and have the Wrath of the Lamb. Achievements added with that paid DLC are listed further down the page.

General Progression

Name/Desc

The Radioactive Spider
- Defeat the Boss at the end of Basement 2.

Monstro's Tooth
- Defeat all types of Boss that can appear in the Basement. These being The Duke of Flies, Monstro, Gemini and Larry Jr.

The Spelunker
- Defeat the Boss at the end of Caves 2.

Lil Chubby
- Defeat all types of Boss that can appear in Caves. These being Fistula, Gurdy, Peep and Chub.

I Killed Mom!
- Defeat the Boss at the end of Depths 2.

Loki's Horns
- Defeat all types of Boss that can appear in the Depths. These being Monstro II, Loki, and Mom.

The Womb

After finishing the boss in The Depths 2 the first time, every other time you beat her you'll be taken to a new floor: The Womb.

Judas' Fez
- Defeat the Boss at the end of The Womb 2.

The Noose
- Defeat the Boss at the end of The Womb 2 a SECOND time.

The Nail
- Defeat the Boss at the end of The Womb 2 a THIRD time.

The Quarter
- Defeat the Boss at the end of The Womb 2 a FOURTH time.

The Fetus
- Defeat the Boss at the end of The Womb 2 a FIFTH time.

Everything's Terrible
- Defeat the Boss at the end of The Womb 2 a SIXTH time.

The Wafer
- Defeat the Boss at the end of The Womb 2 a SEVENTH time.

Money = Power
- Defeat the Boss at the end of The Womb 2 an EIGHTH time.

It Lives!
- Defeat the Boss at the end of The Womb 2 a NINTH time.

???
- Defeat the Boss at the end of The Womb 2 a TENTH time.

The Cross
- Defeat the Boss in The Womb 2 when playing as Maggy.

The Bag
- Defeat the Boss in The Womb 2 when playing as Cain.

The Robo-baby
- Defeat the Boss in The Womb 2 when playing as Judas.

The Razor Blade
- Defeat the Boss in The Womb 2 when playing as Eve.

The D6
- Defeat the Boss in The Womb 2 when playing as ???, also known as The Blue Baby.

Sheol

Upon completing The Womb 2 a tenth time, you can enter a new floor: Sheol.

Mom's Knife
- Defeat the Boss in Sheol when playing as Isaac.

The Guardian Angel
- Defeat the Boss in Sheol when playing as Maggy.

The Bomb Bag
- Defeat the Boss in Sheol when playing as Cain.

The Monster Manual
- Defeat the Boss in Sheol when playing as Judas.

The Demon Baby
- Defeat the Boss in Sheol when playing as Eve.

Forget Me Now
- Defeat the Boss in Sheol when playing as ???, also known as The Blue Baby.

Character Unlock Achievements

To unlock the ability to play as other characters, each of which have different stats and starting items (if any), you need to complete various objectives throughout your time playing The Binding of Isaac.

Name/Desc

Maggy's Wig
- Have a maximum HP of 7 at any one point during a playthrough. This unlocks Magdalene.

Cain's Eyepatch
- Have 55 or more pennies on hand at any one point during a playthrough. This unlocks Cain.

Eve's Dead Bird
- Make 2 deals with the Devil in a single playthrough. This unlocks Eve.
(Note that you don't have to survive the second deal, if you're looking for a quick unlock)

Judas' Fez
- Defeat the Boss at the end of The Womb 2. This unlocks Judas.

???
- Defeat the Boss at the end of The Womb 2 a TENTH time. This unlocks ???, also known as The Blue Baby.

Boss Related Achievements

This, obviously, excludes achievements already listed above. We all know what happens when you beat Basement 2's boss!

Icon

Name/Desc

The Halo
- Kill Mom or Mom's Heart using The Bible. Using The Bible will kill her instantly.

The Bandage
- Kill all four horsemen in a single playthrough.

The Book of Sin
- Defeat all of The Seven Deadly Sins over multiple playthroughs.

My Head Hurts...
- Defeat the Boss Steven. Steven is an alternate form of Gemeni, meaning you encounter him in The Basement.

I Need a Hug...
- Defeat the Boss C.H.A.D.. C.H.A.D. is an alternate form of Chub, meaning you encounter him in The Caves.

I have Sticker Fingers...
- Defeat the Boss Gish. Gish is an alternate form of Monstro II, meaning you encounter him in The Depths.

A Gift from Krampus
- Defeat Krampus in a Devil Room. Krampus appears at random within Devil Rooms.

More Specific Achievements

Basement Boy
- Complete The Basement without taking any damage. Note that the item "We Need to go Deeper" can ONLY be used on the first floor of the two. You MUST defeat the second floor boss.

Spelunker Boy
- Complete The Caves without taking any damage. Note that the item "We Need to go Deeper" can ONLY be used on the first floor of the two. You MUST defeat the second floor boss.

Dark Boy
- Complete The Depths without taking any damage. Note that the item "We Need to go Deeper" can ONLY be used on the first floor of the two. You MUST defeat the second floor boss.

Mama's Boy
- Complete The Womb without taking any damage. Note that the item "We Need to go Deeper" can ONLY be used on the first floor of the two. You MUST defeat the second floor boss.

Got the Wrath of the Lamb DLC? - If attempting the "Iron-Boy" achievements on XL-floors "We Need to go Deeper" CANNOT be used.

The Rock
- Destroy 50 rocks. This is counted over multiple playthroughs!

The Bean
- Destory 50 poo piles. This is counted over multiple playthroughs!

Mom's Pill Bottle
- Use Blood Donation Machines 20 times. This is counted over multiple playthroughs!

The Gamekid
- Enter different Arcades 10 times. This is counted over multiple playthroughs!

Mr Mega!
- Destroy 30 X Marks the Spot rocks. This is counted over multiple playthroughs!

The Necronomicon
- Use the tarot card XIII Death four times. This is counted over multiple playthroughs!

The Shears
- Die 100 times. :(

The Common Cold
- Collect two of these three items in a single playthrough: Roid Rage, The Virus, Growth Hormones.

The Parasite
- Collect two of these four items in a single playthrough: Bob's Rotten Head, Guppy, Max's Head, Tammy's Head.

Mom's Contacts
- Collect Mom's Eye after collecting any other "Mom" Items in a single playthrough.

Golden God
- Collect Every item in the game. If you have defeated Krampus and picked up the Lump of Coal, you can earn this achievement even if you are missing an item. This is because Krampus and the Lump of Coal were added in a later update, but this achievement was not updated to reflect this.

Eternal Achievements

The Eternal achievements are for completing various aspects of the game with "Hard Mode" enabled.

Eternal Mom
- Defeat the boss of The Depths 2 while playing in Hard Mode.

Eternal Heart
- Defeat the boss of The Womb 2 while playing in Hard Mode.

Hard Game
- Defeat the boss of The Chest while playing in Hard Mode. To access The Chest, you need to defeat the boss of the Cathedral while holding the Polaroid trinket.

Eternal Isaac
- Complete the game as Isaac while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Maggy
- Complete the game as Maggy while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Cain
- Complete the game as Cain while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Judas
- Complete the game as Judas while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Eve
- Complete the game as Eve while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Samson
- Complete the game as Samson while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal ???
- Complete the game as the Blue Baby (Also known as ???) while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Personalities
- Complete the game as every character at least once while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Satan
- Defeat the boss of Sheol while playing in Hard Mode.

Eternal Cathedral
- Defeat the boss of The Cathedral while playing in Hard Mode.

Eternal Life
- Up your max health 7 times by using Eternal Hearts. These are the white hearts. Two half-Eternal hearts together on the same floor increases your max health by one. Entering a new floor with a half-Eternal heart will also increase your max health by one.

Eternal God
- 100% the game, with Hard Mode enabled. HAVE FUN!

Wrath of the Lamb DLC
