Welcome fellow Achievement Junkie... At least that's why I'm going to assume you're here! If you're having trouble with figuring out any achievements within The Wrath of the Lamb DLC for The Binding of Isaac, this guide is here to tell you the requirements for each one!
Sure, all of this information is on the unofficial Isaac Wikia, but not everyone knows about that and being quite the contributor over there I thought it'd be worth bringing the guide over here, with a few changes here and there! :)

If you're looking for details regarding a specific achievement, Ctrl+F is your best friend.

Samson

Samson is the playable character added in The Wrath of the Lamb. This section will outline the Samson-related achievements, up to, but not including, Cathedral. The Cathedral section is just below this one!

Icon

Name/Desc

Samson
- Pass two treasure rooms (The golden-framed doors) and defeat The Depths 2 Boss. This unlocks Samson as a playable character.

Blood Lust
- Defeat the Boss in Womb 2 or Utero 2 when playing as Samson.

Blood Rights
- Defeat the Boss in Sheol when playing as Samson.

The Cathedral

If you have defeated the Boss at the end of Womb 2/Utero 2 ten or more times, you'll have the option to go to the Cathedral floor by walking into the beam of light.

- Defeat the Boss of the Cathedral when playing as Isaac.

The Cross
- Defeat the Boss of the Cathedral when playing as Maggy.

Abel
- Defeat the Boss of the Cathedral when playing as Cain.

The Curved Horn
- Defeat the Boss of the Cathedral when playing as Judas.

The Sacrificial Knife
- Defeat the Boss of the Cathedral when playing as Eve.

The Rainbow Baby
- Defeat the Boss of the Cathedral when playing as ???, also known as The Blue Baby.

The Bloody Penny
- Defeat the Boss of the Cathedral when playing as Samson.

The Polaroid
- Defeat the Boss of the Cathedral FIVE times, using any character.

The Chest

Unlocking the Chest Floor

So you've defeated the Boss of the Cathedral five times, unlocked the Polaroid, soldiered on once again and this time you've beaten the Cathedral while holding the Polaroid in your Trinket slot. Good job! Welcome to The Chest. You will need to beat the boss of The Cathedral while holding the Polaroid trinket with you every time to progress to The Chest.
Once the Polaroid trinket has been unlocked it will always drop after defeating the Mom (Depths 2) boss fight.

Dad's Key
- Defeat the Boss at the end of The Chest.

Isaac's Head
- Defeat the Boss at the end of The Chest when playing as Isaac.

Maggy's Faith
- Defeat the Boss at the end of The Chest when playing as Maggy.

Cain's Eye
- Defeat the Boss at the end of The Chest when playing as Judas. This achievement is currently bugged and SHOULD unlock when beating The Chest as Cain.

Judas' Tongue
- Defeat the Boss at the end of The Chest when playing as Eve. This achievement is currently bugged and SHOULD unlock when beating The Chest as Judas.

Eve's Bird Foot
- Defeat the Boss at the end of The Chest when playing as ???, also known as The Blue Baby. This achievement is currently bugged and SHOULD unlock when beating The Chest as Eve.

???'s Soul
- Defeat the Boss at the end of The Chest when playing as Cain. This achievement is currently bugged and SHOULD unlock when beating The Chest as ???, also known as The Blue Baby.

Samson's Lock
- Defeat the Boss at the end of The Chest when playing as Samson.

Challenges

Wrath of the Lamb added a new Challenge mode, with ten challenges to complete. Each challenge unlocks an achievement upon completion.

The Candle
- Complete Challenge 1. This challenge is to complete Depths 2 with the Curse of Darkness being active on every floor.

The Lucky Toe
- Complete Challenge 2. This challenge is to complete Depths 2 while starting with bad luck.

The Burnt Penny
- Complete Challenge 3. This challenge is to complete Depths 2 with the Curse of the Labyrinth on every floor.

Guppy's Tail
- Complete Challenge 4. This challenge is to complete Depths 2 while starting with the Guppy item.

The Fish Head
- Complete Challenge 5. This challenge is to complete Womb 2, or Utero 2, starting with The Mulligan and Infestation items.

Mega Fetus
- Complete Challenge 6. This challenge is to complete Womb 2, or Utero 2, starting with the Dr Fetus and Doctor's Remote items.

Super SMB Fan
- Complete Challenge 7. This challenge is to complete Womb 2, or Utero 2, starting with four cubes of meat.

The Spider Butt
- Complete Challenge 8. This challenge is to complete Womb 2, or Utero 2, starting with the Mutant Spider and Spider's Bite items.

The Counterfeit Coin
- Complete Challenge 9. This challenge is to complete Sheol starting with the Holy Grail and The Halo items.

Daddy's Love
- Complete Challenge 10. This challenge is to complete The Cathedral with no starting item and no Item Rooms (Golden-framed doors).

More Specific Achievements

Guppy's Hairball
- Pick up three of these four items in a single playthrough to transform Isaac into a cat: Dead Cat, Guppy's Tail, Guppy's Head, Guppy's Paw.

The Left Hand
- Defeat the Ultra Pride miniboss.

Platinum God!
- Collect all items and all secrets in the game! It's going to be a long night.

Eternal Achievements

The Eternal achievements are for completing various aspects of the game with "Hard Mode" enabled.

Eternal Mom
- Defeat the boss of The Depths 2 while playing in Hard Mode.

Eternal Heart
- Defeat the boss of The Womb 2 while playing in Hard Mode.

Hard Game
- Defeat the boss of The Chest while playing in Hard Mode. To access The Chest, you need to defeat the boss of the Cathedral while holding the Polaroid trinket.

Eternal Isaac
- Complete the game as Isaac while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Maggy
- Complete the game as Maggy while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Cain
- Complete the game as Cain while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Judas
- Complete the game as Judas while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Eve
- Complete the game as Eve while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Samson
- Complete the game as Samson while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal ???
- Complete the game as the Blue Baby (Also known as ???) while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Personalities
- Complete the game as every character at least once while playing in Hard Mode. "Complete" means to defeat the boss of The Chest.

Eternal Satan
- Defeat the boss of Sheol while playing in Hard Mode.

Eternal Cathedral
- Defeat the boss of The Cathedral while playing in Hard Mode.

Eternal Life
- Up your max health 7 times by using Eternal Hearts. These are the white hearts. Two half-Eternal hearts together on the same floor increases your max health by one. Entering a new floor with a half-Eternal heart will also increase your max health by one.
