first you need the afterbirth DLC
then you have to defeat mother atleast once to unlock moms heart
then you kill the heart 11 times to unluck all normal items so you can get equipment and unlock it lives
after that defeat satan aka the evil guy aka the devil aka the villain from cuphead 5 times
after that you will unlock the polaroid then you can go chose to go to sheol or the cathedral i sugest you do it to get equipment
then start a new run if you use azazel i sugest you look for the item that throws tears everywhere and use the item on now in your run you have to go to it lives in less than 30 minutes and if it is more you cant but if it is less then you can fight against i suggest you bring 3+ keys since you can open the treasure rooms there and get more items and you need to go to the upper room and you will start to fight and then good luck! i hope this helps atleast one person
also fun fact!: hush represents isaac slowly sufocating and the max time of 30 minutes represents the time isaac took to die sufoccated and at his first phase the soundtrack is the same has the one from blue baby another fun fact is that my name is also isaac!
