Step 1

Rewind back to 1993 when the Cacodemon had an unnecessarily large bumhole, and steal this image of it

Step 2

Steal another image (such as an imp) off of the internet

Step 3

Do a bit of photo-shopping and stick the head of the imp (or whatever image you use) into the butthole of the Cacodemon
