First of all, you can complete mission in whatever order u like it really doesn't matter.

Escaped!
Complete Prologue.

This is the very first mission that the game will start you in once upon first playing the game
In addition u can skip it by pressing F2 twice and exiting to main menu.

Virtuoso
Complete a level as every Hero.
As simple as it sounds, complete 1 mission with every character, so Sienna, Kerillian, Kruber and Bardin. In addition u can use whatever career u like on them.
(Mission have to be the exactly this same for every class/character for example "Empires in Flames.")

Complete Act 1
Complete every mission in act 1. Striking Back
Complete Act 2
Complete every mission in act 2.
Reikland Rumble
Complete Act 3
Complete every mission in act 3.
You need to complete 1 mission on 4 difficulty's or simply once on Legend.
The mission that you have to complete is called The Skittergate.
The Frozen North
Complete Skittergate on Recruit. Pact-Smasher
Complete Skittergate on Veteran. Righteous Crusade
Complete Skittergate on Champion. Norscannihilation
Complete Skittergate on Legend.

Equip a Common Item.

Simply equip item with green background otherwise called "Common Item."

Quite the Find
Equip a Rare Item. Simply equip item with blue background otherwise called "Rare Item."
Heirloom
Equip an Exotic Item. Simply equip item with orange background otherwise called "Exotic Item."

Mark of expertise
Equip a Veteran Item. Simply equip item with red background otherwise called "Veteran Item."

Now You're Showing Off
Equip Exotic items in every slot. Simply equip items with orange background on every slot for example.

Craftsman
Craft an Item
Craft 1 item.You can do this by collecting 10 scraps and then crafting item. Master Craftsman
Craft 50 Items
Simply collect more scraps and craft 49 more. Master Craftsman
Salvage an Item
Salvage your first item. Make Do and Mend
Salvage 100 items
Just salvage 99 more.
