Player Spawn Point Mechanics
Overview/How Player Spawns Work
Grim Dawn doesn't offer much information about how player spawn points work or what determines where you will spawn upon loading a game or resurrecting. I have compiled my findings here after spending extensive time in the game and searching this and other forums.
There are seven spawn points in Cairn:
Devil's Crossing
Homestead
Fort Ikon
Coven's Refuge
Malmouth Sewers
Conclave of the Three
Vanguard of the Three
To activate or set a spawn point, your character must run over it. That's it. However, the 'hit boxes' for the spawn points are quite small so you must be precise, or just run in tight circles like I do. This will be that character's spawn location upon loading into the game or resurrecting after dying. I have included images of my character on each spawn point upon loading into the game without moving to show where to walk to activate whichever you choose.
To change your spawn point, simply run over a new one. Conversely, avoid running over these spots when returning to town if you don't want a new spawn point.
Spawn points are unique to each difficulty so if you spawn in Fort Ikon (for example) but didn't go there during your last play session, check the difficulty. Veteran is an enhanced version of Normal, so it shares spawn points with Normal.
All characters will start by the hangman's noose in Devil's Crossing upon creation and when first entering Elite or Ultimate difficulty. Interestingly, the spawn point in Devil's Crossing is enlarged before you activate it for the first time so it's possible to barely enter the gate and quit before reaching the trigger causing you to spawn at the noose when reloading. Once the Devil's Crossing point is activated though, it becomes impossible to spawn at the noose again.
Setting a spawn point is useful for target farming a Monster Infrequent (MI) or running the rogue-like dungeons or Shattered Realm as it avoids tediously scrolling the world map when reloading or dying.

Conclusion
I labeled this as a walkthrough and not as a basics guide because the location names are minor spoilers. If you think I should add that tag, let me know.
