Introduction

Welcome to the guide!

Here, i will be telling you the 101 on the good and bad items for ???. In other words, what items to take and what items to NOT take. Anything that isn't mentioned as either should be up to you. The only exception is the D6 if you unlocked it, that is.

WOTL (DLC) items will be marked.

Enough of my rambling. ON TO THE GUIDE!

The Good Items

Items/Rooms/Content in bold are DLC only.

- If you've unlocked Eve, the "♥♥♥♥♥ of Babylon" item works very well with ???. Since it's supposed to work when you reach half a heart, ??? basically uses it as a permanent passive damage upgrade- He has -1 HP! Only problem is, beating (SPOILERS) The Chest (SPOILERS OVER) will count as beating it as Eve. This is a bug.

-Flying items are very important. Especially with spectral tears (Ouiga board, Dead Dove, and Spirit of the Night.) You can easily prevent damage this way.

-Any item that increases spirit hearts, Such as Book of Revelations, The Nail, The Rosary, Mitre,

-An insane damage dealing item, albeit rare, should never be passed up (obviously).
Good examples would be Dr Fetus, Epic Fetus, Ipecac, Sacred Heart, and Mom's Knife.

-There are a couple items in the DLC that have multiple effects (some giving Spirit Hearts/HP UPs.)
Examples being Fate, Holy Grail, and MEAT!.

-The only good item left is Spoon Bender (Homing Shots)

Now on to the items you should leave alone/reroll.

The Ineffective/Bad Items

Bold items/content are still DLC only.

- Little CHAD (Gives red hearts, which ??? cannot use)

- Charm of the Vampire (Doesn't work at all with ???)

- Guppy's Paw (No red hearts to transform)

- Yum Heart (Only heals red hearts, which ??? has none of)

- The Gamekid (Doesn't give you soul hearts, making it exactly the same as a unicorn horn.)

- The Blood Bag (Only gives one spirit heart as ???, healing nothing but giving one heart, basically making it not worth the HP you might need to get it)

- IV Bag (You really don't have enough health EVER for this one.)

Useless Trinkets (DLC ONLY)--------------------------------------------------------------

Bloody Penny - It gives you red hearts when you pick up coins - Useless as ???.

Isaac's Fork - Same as Charm of the Vampire.

Child's Heart - Increases spawn chance of Red Hearts. If anything, this reduces the chance of blue/spirit hearts. It's actually negative as ???.

Umbilical Cord - Only works when ??? has no HP at all. Useless.

The Tick - Damages bosses, but it won't heal ???. Not worth it's price.

Cursed Skull - Restarts a room every single time you get hit. Painful.

Useless Tarot Cards (Not entirely DLC)----------------------------------

2 of Hearts - Doubles RED hearts. Won't work with ???'s blue hearts.

VI The Lovers - Spawns 2 red hearts. Useless while playing as ???.

XIX The Sun - Clears the map screen and does damage to enemies in the room, but it won't heal ??? at all.
