List of MI Locations (Updated for Fangs of Asterkarn)
What the heck is an MI?
Some Monsters have a small chance to drop equipment that is specific to them, and only them.
Great. Why should I care?
Epic and Legendary MIs:
Epic MIs can be useful for leveling depending on your build, and the appeal of the boss-only Legendary drops are probably self explanatory.
But what about....
Rare (Green) MIs:
Depending on the build, getting an MI with a rare prefix and a rare suffix that both benefit the build, can elevate an MI to Best in Slot.
And barring that, Some of them are crafting materials too.

I used a mix of Grim Tools and the wiki to make a list for myself, since I prefer having a printout in front of me for reference instead of opening GrimTools 24/7.
I listed whatever GrimTools had tagged as an 'MI'.
As for Location, I just picked one or two places that had the highest enemy spawn count for that drop.

Note 2: Hidden Path Questline
Thingy 1) NE of Lower Crossing Rift, look for opening to river, 2 dynamite needed
Thingy 2) SW of Broken Hills rift on long, thin path. look for the large rock to click on
Thingy 3) North of Asterkarn Road rift. Right when Asterkarn Valley text changes to road, look for the large rock to click. Cave has guardian and a hidden second room with a Base Game Superboss inside.
→ Temple of the Three located in East Marsh (East of Burrwich Village rift) Head to the bridge without crossing it, then head SE and look for the large rock squished in a clump of trees.
Note 3: Finding the Tomb of the Heretic is an Act 7 questchain:
Seal 1) see maw ** above
Seal 2) Secret path south of korvan sands rift (near 2 griffin statues)
Seal 3) Secret path in sunbane oasis → howling chasm. look for false wall
→ key dungeon SE of Korvan City rift in Valley of Chosen
(*1, *3): Lokarr Quest Chain (Ultimate Difficulty Only)
SW of Gloomwald Crossing Rift, detonate the rocks as part of Nate’s Stash Quest (Find a note in one of the houses in Gloomwald to initiate, detonation site won’t appear without it) In the Stash area, head north into hidden room with a corpse and a blacksmith, Arcturus.
Loot and read the “Turn Back” note from the corpse. Talk to Arcturus and craft the Heart of Darkness.
North of Barrowholm Rift → Altar of Rattosh → Desolate Wastes → go all the way NE, find and break the “ancient urn” to get the “Myhthical Stormheart”
South of Malmouth Outskirts Rift, near the shrine, drag blue zombie thing “Flameond Bleu” to the blue circle there.
For pet builds: Clear area around shrine, dismiss pets, then look for the zombie. (There’s extra babysitting involved since the pet AI is too dumb to not kill the squishy quest zombie.)
Save yourself the headache: reset the game until you get the SE spawn otherwise you’ll just be getting door-blocked and ganked the entire time. Blue zombie is decently fast, so at least in the SE spawn you can usually kite it fast enough across the bridge to avoid the rest of the gank squad.
Go through the new rift, talk to the NPC then Go to Devil’s Crossing Rift and check the green crashed thingy near the shop.
Go back to the green rift NPC and give her: the heart of darkness, the mythic stormheart, and the thingy from devil’s crossing. (You get to keep the mythic stormheart)
Sanctum of the Chosen → Hidden door behind shrine. Rift-Claimed Adherent’s will be inside this area. To fight Lokarr: Find the second hidden area → look for the arch leading into a blood pool and then enter the tree → kill the ‘root of all evil’ to open a rift to him.
Lokarr says hi. Kill him! :)
*(2) Clone Questchain (Ultimate Difficulty Only)
Dalia’s Diary: West of Homestead Rift in a house, give it to Dalia Thornsbury in Homestead.
Asterkarn Valley → Tomb of Korvaak head east, then to the SW room and destroy the urn in the corner to get “Stormheart” Keep it in your inventory until quest is done.
Sorrows Bastian → Head NE into blood grove, hugging the edge until you find an Oak Tree. Read the Note.
Steps of Torment key dungeon → Anguish, break a wall, stand between the pillars and read the note.
- Plains of Strife, red rift-> Go NE, find another letter in the house
Make a new character, put their “Salt Bag” in shared stash, then delete the character.
Hidden area NE of Gates of Necropolis Rift, open the portal with the salt bag
Click on loose torch north of the shrine.
Clone says hi. Kill!
*(4) FG Superboss Questchain (Ultimate Difficulty Only)
Vanguard of the three → green circle → ‘Chillheart’
Seal 1: Conclave of the Three East side of map → hidden cellar, break the crate
Seal 2: Temple City of Osyr: Hidden Area North of Rift, break the crate
Seal 3: Cairan Docks SE of Rift, follow the river under the bridge, break the crate
Seal 4: NE of Vanguard of the Three Rift → break the wall, break the crate
Seal 5: SE of Korvan City Rift → break the wall, break the crate
Seal 6: Tomb of Eldritch Sun 2nd floor, break the wall, break the crate
→ North of Korvan Sands Rift → Bonebleach Basin → Edge of Reality:
Lever Order: White, Yellow, Green, Blue, Purple
Kill the “Perfectly Usual Oversized Maggot” and go through the portal.
This thing exists. Better fix that: Kill it!

Note: You can reset the key dungeon Merchants’ inventory by changing floors. You don’t have to leave the dungeon.
To change Non-key dungeon Merchant Inventories, teleport to an area that is far enough away from their location and either, wait 10+ seconds, or talk to two or three other merchants before returning to check their inventory again.
Agarrad (Port Valbury Key Dungeon - HighTown)
Benevald is located in Blood Grove. From the blood grove rift, head south past the bridge, then hug the edge and go right following the river. There’s a gap in the trees you can pass through into the river and through a pipe under the bridge. Benevald is on the island past there.
Hyram (Steelcap District after completing ‘Liberate the Liberators’ quest)

As of the v1.2.1.0 Patch, Nemesis’ no longer have set spawn locations.
Once you reach that reputation level, they have a chance to spawn when killing members of their faction.
They can also only spawn once per session.
I’ll keep the area listings as they were for reference, since most of the old areas listed have a ton of that faction there anyway.

If it's anything like the other super bosses, then there's probably quest chains that need to be completed in the hardest difficulty.

Judging by the Nemesis changes from v1.2.1.0 these new ones probably also don't have set spawn locations and instead only appear on killing members of their factions. (maybe?)
There's probably a lot of new factions that they are a part of that we don't have the names of yet.
We'll have to see. Not sure yet.

It's also noted as being Celestial. Not sure about this one either.
So, either this nemesis actually DOES have a set location --making me wonder if the other new nemesis' do too, despite the '?' listings /if they still just need max rep faction kills-- or if this boss is part of a quest or something. Not sure.
Also, a few of the grim tools MI listings for the expansion had some AOM, FG, etc, MI's sprinkled in.
