Weapon soft-lock Glitch

There is a bug that occurs when you're rapidly firing your Pistol and throw a Grenade that soft locks your game and you can no longer use any weapons. Dying or manually reloading from previous check point allows you to continue, but simply entering the inventory (tab on PC) will unlock your weapons allowing you to continue uninterrupted. I ran into this alot early game where the pistol is the primary weapon.

You can avoid this simply by letting a brief second pass after tossing the grenade before opening fire again, or throwing in a quick melee hit before firing.
